肝动脉灌注栓塞化疗联合射频消融和无水乙醇化学消融治疗原发性肝癌的临床疗效分析

摘　　要：目的探讨多种手段联合治疗对原发性肝癌的临床效果。方法 90例确诊为原发性肝癌患者随机分为对照组和实验组,对照组采用常规射频消融联合肺动脉灌注栓塞,实验组在对照组的基础上加用无水乙醇化学消融治疗,对比分析2组患者治疗效果。结果对照组患者有效率为57.8%,实验组患者有效率为80.8%,2组差异具有统计学意义(P<0.05)。实验组患者治疗后6个月、12个月以及24个月的存活率分别为93.3%、82.2%和42.2%,显著高于对照组,差异具有统计学意义(P<0.05);实验组患者治疗后肿瘤完全坏死率为77.8%,显著高于对照组的20.0%,差异具有统计学意义(P<0.05)。2组患者不良反应发生率差异不具统计学意义(P>0.05)。结论肝动脉灌注栓塞化疗联合射频消融和无水乙醇化学消融对原发性肝癌的临床治疗效果显著,值得临床推广。Objective To investigate the clinical therapeutic effect of combined treatment of multiple means for primary liver cancer. Methods 90 patients with primary liver cancer were randomly divided into the control group and the experimental group. The control group was treated with conventional radiofrequency ablation combined with pulmonary arterial embolization. The experimental group was treated with ethanol chemicalablation on the basis of the control group. The treatment effect was compared between the 2 groups. Results The effective rate of the control group was 57. 8%,and the effective rate of the experimental group was 80. 8%. The difference was statistically significant（ P〈0. 05）. 6-,12-and 24-month survival rates of the experimental group were 93. 3%,82. 2% and 42. 2%,respectively,which were significantly higher than those of the control group（ P〈0. 05）.The complete necrosis rate of the experimental group was 77. 8%,which was significantly higher than 20. 0% of the control group,and the difference was statistically significant（ P〈0. 05）. There had no significant differences in adverse reactions between the 2 groups（ P〈0. 05）. Conclusion Hepatic arterial chemoembolization combined with radiofrequency catheter ablation and ethanol chemical ablation for primary liver cancer has significant clinical efficacy,and it is worthy of clinical promotion.
