摘 要:目的探讨SPECT/CT骨显像对脊柱良恶性病变所致放射性浓聚的鉴别诊断价值。方法选取行^(99m)TcMDP全身平面骨显像提示放射性浓聚的患者46例,72个脊柱病灶,对其进行SPECT/CT断层显像,并与平面显像进行比较。所有病灶的最终诊断结果均以磁共振、病理或6个月以上的随访结果作为标准。结果最终诊断中46例患者共有病灶72个,其中骨转移灶59个,良性病灶13个。SPECT/CT骨显像诊断脊柱病灶的特异度、准确度、阳性及阴性预测值均明显高于^(99m)Tc-MDP全身平面骨显像(P<0.05),两种方法灵敏度比较差异无统计学意义(P<0.05)。结论SPECT/CT骨显像可有效提高脊柱病灶的诊断特异性和准确性,在脊柱良恶性病变所致放射性浓聚的鉴别诊断中具有较高的临床价值。Objective To explore the differential diagnosis of SPECT/CT on spinal benign and malignant lesions caused radioactivity. Methods Line 99 MTC-MDP general planar bone scintigraphy prompt radioactive concentration of 46 cases of patients,72 spinal lesions received SPECT / CT tomography examination,and comparing with planar imaging. All final diagnosis lesions in MRI,pathology,or more than 6 months of follow-up results as standard. Results Eventually there were altogether 72 lesions,diagnosis of 46 patients with 59 bone metastases,13 benign lesions. SPECT / CT imaging in the diagnosis of spinal bone lesions of speciality degree,accuracy,positive and negative predictive value were significantly higher than 99 MTC-MDP general planar bone scintigraphy( P〈0. 05),there had statistical difference( P〈0. 05). Conclusion SPECT / CT on spinal benign and malignant lesions can effectively improve the specificity and accuracy of diagnosis of spine lesions,benign and malignant lesions caused by radioactive concentration in spine has high clinical value in differential diagnosis.
