359例小肠胶囊内镜检查的回顾性分析

摘　　要：目的探讨胶囊内镜对小肠疾病的诊断价值。方法对行OMOM型胶囊内镜检查的359例患者的临床资料进行回顾性分析。结果 346例(96.4%)患者的胶囊内镜顺利通过回盲部;125例(34.8%)患者因1 h未能通过幽门用胃镜圈套将胶囊送入十二指肠。胶囊内镜阳性病变233例(64.9%)。54例不明原因消化道出血患者中,检出阳性病变42例(77.8%),其中血管病变24例,溃疡病变12例,寄生虫1例,小肠息肉2例,小肠憩室3例;189例腹痛患者中,阳性病变135例(71.4%),其中糜烂性病变71例,溃疡性病变37例,炎症反应性病变14例,血管性病变9例,寄生虫3例,小肠息肉1例;105例腹泻患者中,发现阳性病变55例(52.4%),其中糜烂性病变19例,溃疡性病变16例,血管性病变11例,黄斑瘤5例,小肠息肉4例。结论胶囊内镜对小肠疾病具有较高的诊断价值,特别是对小肠炎症性疾病以及不明原因消化道出血的临床诊断。Objective To investigate the value of capsule endoscopy in the diagnosis of small bowel diseases. Methods The clinical data of 359 cases underwent OMOM capsule endoscopy examination were retrospectively analyzed. Results In 346 patients（ 96. 4%）,capsule endoscopy was successfully passed the ileocecal valve; because the capsule endoscopy had failed to pass pylorus after 1 h,the capsule endoscopy were sent to the duodenum by using pyloric duodenal endoscopy in 125（ 34. 8%） patients. Positive lesions were found by capsule endoscopy in 233 cases（ 64. 9%）. Among 54 cases of unexplained gastrointestinal bleeding patients,positive lesions were detected in 42 cases（ 77. 8%）,including 24 cases of vascular disease,12 cases of ulcer lesions,1 case of parasite,intestinal polyps（ 2 cases）,small intestinal diverticulum（ 3 cases）. Among 189 cases of abdominal pain group,positive lesions in 135 cases（ 71. 4%）,including71 cases of erosive disease,ulcerative lesions in 37 cases,inflammatory lesions in 14 cases,9 cases of vascular lesions,a parasite in 3 cases,1 cases of small intestinal polyps. Among 105 cases of diarrhea,positive lesions were found in 55 cases（ 52. 4%）,including 19 cases of erosive lesions,16 cases of ulcerative lesions,11 cases of vascular diseases,5 cases of macular tumor,4 cases of small intestinal polyps.Conclusion The diagnostic value of capsule endoscopy for small intestinal disease is rather high,especially for intestinal inflammatory disease and obscure gastrointestinal bleeding in clinical diagnosis.
