摘 要:目的通过研究细胞动力学标志物胸苷激酶1(TK1)与乳腺癌前病变进程之间的相关性,评价其在癌前病变诊断准确性和癌前疾病恶性进展评估方面的应用价值。方法采用TK1抗体对3 442例女性体检者进行血清TK1水平检测,其中乳腺病变组1 205例,健康组2 237例,并对其中90位乳腺癌前病变患者进行了长达3年的跟踪,研究TK1细胞增殖分子与乳腺癌前病变进程之间的相关性;并结合检测对象病情的进展情况,对TK1评估的准确性进行分析。结果健康组TK1平均水平与乳腺病变组比较,差异有统计学意义(P〈0.05);同时,对比乳腺癌前病变的年龄分布,观察到全年龄段乳腺癌前病变发生率与TK1无相关性(r=0.491,P〉0.05)。30岁后二者有相关性(r=0.933,P〈0.05),乳腺癌前病变中重度导管内乳头状瘤和非典型增生患者TK1水平高于单纯增生(P〈0.05)。当TK1为1.4 pmol/L时区分乳腺良恶性病变的特异性为89.5%,灵敏度为71.1%。对90例癌前病变患者进行6~36个月的跟踪,癌前病变进展组患者,TK1水平复检时较初检升高,差异有统计学意义(P〈0.05)。结论 TK1对于乳腺癌前病变患者恶性进程风险预测具有重要意义。Objective To investigate the correlation between thymidine kinase 1( TK1) and the process of precancerous lesion of breast cancer in order to evaluate the diagnostic accuracy and the utilization value of thymidine kinase 1in the evaluation of malignant process of precancerous lesion of breast cancer. Methods The serum levels of TK1 were detected by using TK1 antibody in 3,442 female subjects,including 1,205 cases of mammary gland lesions( mammary gland lesion group) and 2,237 healthy subjects( control group),moreover,of whom 90 patients with precancerous lesions of breast cancer were followed up for 3 years to investigate the correlation between thymidine kinase 1 and the process of precancerous lesion of breast cancer,furthermore,to evaluate the diagnostic accuracy of thymidine kinase 1 in precancerous lesions of breast cancer. Results There were significant differences in the average levels of TK1 between control group and mammary gland lesion group( P 0. 05). Moreover,the incidence of precancerous lesion of breast cancer was not related with patient's age( P 0. 05). The levels of TK1 in patients with severe intraductal papilloma or atypical hyperplasia were significantly higher than those of patients with simple hyperplasia( P 0. 05). When TK1 was 1. 4pmol / L,the specificity in distinguishing benign lesion from malignant lesion of mammary glands was 89. 5%,and sensitivity was 71. 1%. After 6 ~ 36 months followup,in 90 cases of precancerous lesion of breast cancer,the levels of TK1 in patients with precancerous lesion progression in secondary inspection were significantly increased,as compared with those of patients in initial inspection( P 0. 05).Conclusion TK1 detection plays an important role in predicting the malignant process risk of precancerous lesions of breast cancer.
