摘  要:目的:探讨术前口服碳水化合物对大鼠术后胰岛素抵抗的改善作用及其机制。方法:采用SD大鼠作为实验模型,分别术前给予不同剂量碳水化合物溶液Pre Op,然后进行腹部外科手术并缝合,检测不同的术前处理对术后大鼠空腹血糖和胰岛素浓度、HOMA指数(胰岛素抵抗指数、敏感指数和分泌指数)的影响。采用Western blotting检测大鼠骨骼肌IRS-1、GLUT4、PI3K/Akt的蛋白表达及AKT磷酸化水平。结果:手术组大鼠的空腹血糖升高,胰岛素水平下降;而术前口服碳水化合物能够降低空腹血糖,增加胰岛素含量;这一趋势与HOMA指数结果一致。手术组大鼠的IRS-1、GLUT-4、PI3K表达和AKT磷酸化水平均出现下降,而术前口服碳水化合物能够在一定程度上逆转这一趋势,增强IRS-1、GLUT-4、PI3K表达和AKT磷酸化。结论:术前口服碳水化合物能够改善大鼠术后胰岛素抵抗,可能通过增强IRS-1和GLUT-4表达并促进PI3K/AKT通路的激活,这可能是术前口服碳水化合物改善术后胰岛素抵抗的重要作用机制。Objective: To explore the effects of preoperative oral carbohydrate on the improvements of postoperative insulin resistance in rats and its possible mechanisms. Methods: SD rats were used as experimental models. Before surgery SD rats were treated with different doses of carbohydrate solution Pre Op,then abdominal surgery and sutures were performed. The concentration of plasma glucose and insulin,the HOMA index( insulin resistance index,sensitivity index and secretion index) were measured,respectively. The expression of IRS-1,Glu T4,PI3 K,AKT and p-AKT were examined by Western blotting. Results: The level of glucose was increased and insulin was reduced in the surgical group,and preoperative oral carbohydrate reduced fasting blood glucose,and increased insulin levels; this trend was consistent with the results of HOMA index. In addition,the expression of IRS-1,GLUT-4,PI3 K and AKT phosphorylation was reduced by surgery,which was reversed by preoperative oral carbohydrate. Conclusion: Preoperative oral carbohydrate can improve postoperative insulin resistance in rats,which may be related with IRS-1 and GLUT-4 expression and PI3 K/AKT pathway,thereby improving insulin resistance by preoperative oral carbohydrate.
