膀胱移行细胞癌静脉泌尿系造影检查的临床意义

摘　　要：目的观察探讨膀胱移行细胞癌患者常规行静脉泌尿系造影检查的临床意义。方法选取我院2013年1月至2014年12月收纳的39例膀胱移行细胞癌患者,回顾性分析静脉泌尿系造影检查的结果及临床资料,探讨静脉泌尿系造影检查及B超对于膀胱移行细胞癌患者检出率及影响因素。结果膀胱移行细胞癌在显影上表现为充盈缺损,B超显示为强回声团。其中13例(33.33%)静脉泌尿系造影检查,由于受到肿瘤大小位置和膀胱充盈程度的影响,并未在静脉泌尿系造影检查中发现肿瘤存在。3例(7.69%)B超不能分辨出肿瘤。B超对于膀胱移行细胞癌的显影度明显高于静脉泌尿系造影检查,检出率为92.31%,远超过66.67%,具有统计学意义(P<0.05)。静脉泌尿造影对于肿瘤的显影度受到肿瘤直径、膀胱充盈度与肾脏功能等因素的影响。结论膀胱移行细胞癌患者常规行静脉泌尿系造影检查的检出率低于B超,且具有一定的局限性。但静脉泌尿系造影可以很好的显示整个泌尿系统,对于双侧肾功能也具有诊断价值,可临床进一步推广应用。Objective To observe the clinical significance of the routine use of the bladder transitional cell carcinoma patients with the routine intravenous line of the urinary system. Methods Thirty nine cases of bladder transitional cell carcinoma received by our hospital from January 2013 to December were retrospectively analyzed. clinical data of patients with bladder transitional cell carci-noma were retrospectively analyzed. Results Bladder transitional cell carcinoma showed filling defect in the development, and the ul-trasound showed a strong echo. 13 cases （33. 33%） of the urinary system were examined by the location of tumor size and the filling degree of bladder. 3 cases （7. 69%） B could not be distinguished from tumor. The development of bladder transitional cell carcinoma was significantly higher than that of the urinary system, and the detection rate was 92. 31%, far more than 66. 67%, with statistical significance （P〈 0. 05）. The development of the tumor is affected by the tumor diameter, the filling degree and the kidney function. Conclusion The detection rate of routine intravenous urinary system in patients with bladder transitional cell carcinoma is lower than that of B ultrasound, and has some limitations. But it can be very good to display the whole urinary system, and it also has the diagnos-tic value for the bilateral renal function, which can be further popularized and applied in clinic.
