儿童肺吸虫病7例临床分析

摘　　要：目的：分析我省镇雄县儿童肺吸虫病的临床特征,误诊原因,以提高临床医师对该病的诊治水平.方法：收集2014年9月至2015年3月昆明市儿童医院确诊的7例儿童肺吸虫病患儿,分析其临床特点及诊治经过.结果：7例患儿均来自昭通地区镇雄县,其中女2例,男5例,年龄3～ 14岁,病程6天至半年.7例患儿均长期饮用山泉水,有2例有明确生食螃蟹的病史.7例肺部CT均有肺部渗出性病变及胸腔积液,1例除胸腔积液外合并有中等量心包积液,4例有皮下结节.7例患儿外周血嗜酸性粒细胞绝对值及百分比均明显增高（最高达72.3％）,肺吸虫病血清抗体均阳性.7例患儿予吡喹酮口服治疗后效果好,预后良好.结论：儿童肺吸虫病发病率低,表现多样,尤其是有肺部渗出及胸腔积液时易误诊为结核病,详细询问病史,结合外周血嗜酸性粒细胞增高、免疫学检查可做到及时诊断,正确治疗.Obiective To analyze the clinical characteristics,of children who came from ZhenXiong Yunnan province with paragonimiasis and the reason of misdiagnosis in order to enhance the level of clinical diagnosis and treatment. Methods The clinical data of 7 children with paragonimiasis from September 2014 to March 2015 in our hospital were collected retrospectively and the clinical characteristics and diagnosis and treatment were analyzed. Result 7 children（Sboys and 2 girls）were from 3 to 14 years and they all came from ZhenXiong Yunnan province. The disease course ranged from 6 to 180 day, 7 children often drank raw stream ,two of them had eaten freashwater crab. The CT scan of chest of all the case reveal patchy infiltrates or radiolucencies ,and pleural effusion.. I case had both pleural effusion and pericarditis. There were 4 cases of subcutaneous nodules.Peripheral blood eosinophilia of 7 cases increased .The paragnnimiasis serum antibody were positive in all the cases.The 7 children were cured by using praziquantel and had good prognosis. Conclusion The clinical manifestation of paragonimiasis is complex, so the clinical diagnosis is difficult.Inquiring the patients in detail about history,especially the eating habits of life,and eosinophil count,radiologic findings all play an important role in avoiding mlsdiagnosis.
