摘 要:目的:探讨磁共振成像(MRI)对产前胎盘植入(PIA)的诊断价值。方法:分析18例产前PIA患者的MRI资料,记录MRI检查时显示的胎盘形态、位置、信号均匀程度及子宫壁、相邻器官受累情况,将手术或病理诊断结果与产前MRI图像特征进行比较,评价MRI对产前PIA的诊断符合率及漏诊率。结果:14例MRI产前诊断PIA,与手术或病理诊断结果一致,符合率77.8%(14/18),MRI影像主要表现为胎盘、子宫交界不清,肌层内T2WI高信号带,胎盘组织内出现T2WI低信号带,胎盘内或其下多发迂曲扩张血管影,子宫下段局限性膨隆;4例MRI产前未能诊断PIA,与手术或病理诊断结果不一致,漏诊率22.2%(4/18),均属于粘连型PIA。结论:产前MRI对PIA具有一定的临床诊断价值,可作为超声检查的有益补充。Objective:To investigate the diagnostic value of prenatal MRI in placental implantation abnormality( PIA). Methods:MRI image data of 18 cases of prenatal PIA were analyzed,and the co-incidence rate and rate of missed diagnosis for PIA were analyzed and compared according to the results of operation and pathological diagnosis. Results:14 cases with PIA were diagnosed by prenatal MRI, which was consistent with the results of surgery or pathological diagnosis,and the coincidence rate was 77. 8%(14/18). In these cases,the MRI image was featured by vague placenta-uterine junction, T2WI high signal band in the muscular layer,T2WI low signal band appearing in placental tissue, multiple circuitous angiography in or under placenta and localized swelling of the lower uterine seg-ment. 4 cases with PIA were failed to diagnosed by prenatal MRI,which was not consistent with the results of surgery or pathological diagnosis. The missed diagnosis rate was 22. 2%(4/18),and the prenatal MRI did not show any sign of placenta implantation abnormality. Conclusion:Prenatal MRI has an important clinical value in the diagnosis of placenta implantation abnormality.
