心理干预对中年脑卒中患者临床康复的影响

摘　　要：目的：探讨心理干预对中年脑卒中患者的临床疗效。方法将132例中年脑卒中患者按照随机数字表法分为观察组和对照组，两组患者均给予脑血管疾病常规治疗及护理，观察组同时进行综合性心理干预，观察6周。采用脑卒中临床神经功能缺损程度评分量表评定神经功能康复效果，自编调查表统计患者对疾病恢复的信心、对康复训练的主动性及肌力恢复状况，比较两组患者的护理满意度。结果心理干预6周末观察组脑卒中临床神经功能缺损程度评分量表评分显著低于对照组（P＜0．01），患者对疾病康复的信心、对康复训练的主动性有效率及对护理满意率显著高于对照组（P＜0．01），肌力恢复状况显著优于对照组（P＜0．05）。结论对中年脑卒中患者在常规治疗及护理的同时联合综合心理干预具有增效作用，有利于患者尽快稳定情绪，积极配合治疗，主动积极进行康复训练，对促进患者的全面康复具有重要作用。Objective To explore the effects of psychological intervention on the clinical rehabilitation of middle‐aged patients with stroke .Methods A total of 132 middle‐aged patients with stroke were randomly divided into observation and control group ,both groups received routine treatment and nursing for cere‐brovascular diseases , and observation group was plus comprehensive psychological intervention for 6 weeks . Functional rehabilitation results were assessed with the Clinical Neurological Deficit Scale （CNDS） ,patients’ confidence to recovery ,activeness of rehabilitation training and myodynamia recovery added up using self‐made questionnaire ,and satisfactions to nursing compared between two groups .Re‐sults At the end of 6 week psychological intervention the CNDS score was significantly lower （P〈0 .01） , patients’ confidence to recovery , activeness and effective rate of rehabilitation training were higher （P〈0 .01） ,and myodynamia recovery was better （P〈0 .05） in observation than in control group .Conclu‐sion Routine treatment and nursing plus comprehensive psychological intervention have a synergism for cerebrovascular diseases ,are beneficial to rapid stabilization of emotions ,active coordination with treat‐ment and initiative and active rehabilitation training have an important significance for patients ’ compre‐hensive rehabilitation .
