摘  要:目的检测TTF-1、napsin A和SP-A在穿刺活检标本中肺源性腺癌及非肺源性腺癌的表达,探讨联合应用这3个抗体对判断转移性肺腺癌的意义。方法采用免疫组化En Vision法检测207例穿刺及气管镜活检肿瘤组织(其中原发性肺腺癌83例、转移性肺腺癌79例,非肺来源转移性腺癌45例)中TTF-1、napsin A和SP-A的表达。结果 TTF-1在原发性肺腺癌、转移性肺腺癌和非肺来源的转移性腺癌中表达率分别为88%(73/83)、78.5%(62/79)、28.9%(13/45);napsin A的表达率分别为79.5%(66/83)、75.9%(60/79)、11.1%(5/45);SP-A的表达率分别为68.7%(57/83)、59.5%(47/79),4.4%(2/45),三者在原发性肺腺癌、转移性肺腺癌和非肺来源的转移性腺癌表达之间差异显著(P<0.05)。TTF-1、napsin A和SP-A在肺腺癌表达的特异性分别为71.1%、88.9%和95.6%。结论 TTF-1、napsin A和SP-A联合应用对判断转移性肺腺癌有重要意义。Objective To study the value of integrated use of TTF-1, Napsin-A and SP-A in distinguishing pulmonary adenocarcinoma from non- pulmonary adenoearcinoma in core needle biopsy specimen. Methods The expression of TTF-1, Napsin-A and SP-A was analyzed by immunohistochemistry in 207 cases of lung cancer by bronchoscopic biopsy, including 83 primary pulmonary adenocarcinomas, 79 metastatic pulmonary adenocarcinoma, and 45 metastatic adenocarcinoma from other organs. Results The positive rates of TTF-1 in primary pulmonary adenocarcinomas, metastatic pulmonary adenocarcinoma and metastatic adenocarcinoma from other organs were 88.0% (73/83) , 78.5% (62/ 69) and 28.9% (13/45) ; those of napsin-A were 79. 5% (66/83) , 75.9% (60/79) and 11.1% (5/45) ; those of SP-A were 68.7% (57/83) , 59.5% (47/79) and 4. 4% (2/45) , respectively. There were significantly different expression of TTF-1, Napsin-A and SP-A among them ( P 〈 0. 05 ). The specificity in the expression of TTF-1, Napsin-A and SP-A was 71.1% , 88.9% and 95.6%, respectively. Conclusion There is an important significance in identifying metastatic adenocarcinomas originated from lung by integrated use of TTF-1, Napsin-A and SP-A.
