摘  要:目的研究乌司他丁对纵隔肿瘤切除患者术后认知功能障碍(POCD)的影响。方法选取纵隔肿瘤切除手术的患者60例,采用随机数字表法,将其分成研究组和对照组各30例。研究组患者采用乌司他丁结合多奈哌齐治疗,对照组患者采用多奈哌齐治疗,比较2组患者生存的质量、情绪控制和症状的改善情况,以及患者认知功能评分和POCD的发生率。结果治疗后,研究组患者生存质量,情绪控制,症状改善效果均优于对照组(P<0.05);研究组MMSE、视觉再生、联想学习和数字符号方面得分均优于对照组,差异有统计学意义(P<0.05)。研究组和对照组POCD的发生率分别为6.67%(2/30)以及36.67%(11/30),差异有统计学意义(P<0.05)。结论乌司他丁对纵隔肿瘤切除患者手术后的认知有着明显的改善作用,临床上值得推广研究。Objective To study the effect of ulinastatin on postoperative cognitive dysfunction( POCD) in elderly patients with mediastinal tumor resection. Methods Sixty patients receiving mediastinal tumor resection were randomly divided into study group and control group with 30 cases in each group. Patients in the study group were treated with ulinastatin combined with donepezil,while the control group treated with donepezil. The quality of life,emotional control,and improvement of symptoms,cognitive function assessment of patients,and the occurrence of POCD were assessed and compared between two groups. Results After treatment,the quality of life of patients,emotional control,improvement of symptoms in study group were better than those in control group( P〈0. 05); the scores of MMSE,visual reproduction,associative learning and digit symbol in the study group were better than those in the control group( P〈0. 05). The incidence rate of POCD in study group( 6. 67%) was significantly lower than that of control group( 36. 67%)( P〈0. 05).Conclusions Ulinastatin can improve the cognitive function of elderly patients undergoing mediastinal tumor resection surgery.
