摘 要:目的探讨非小细胞肺癌(NSCLC)患者血浆OLFM4水平及其临床意义。方法收集本院确诊的272例NSCLC患者的外周血样本(NSCLC组),采用酶联免疫吸附法检测其血浆OLFM4水平,分析血浆OLFM4水平与临床病理参数(性别、年龄、肿瘤大小、分化类型、临床分期、T分期、吸烟史和淋巴结转移)的关系;选取同期300例健康体检者的外周血样本作对照。检测156例NSCLC组织样本中OLFM4水平,采用Person相关分析法分析组织与血浆中OLFM4水平的相关性。采用受试者工作特征曲线(ROC)评价血浆OLFM4水平在NSCLC早期诊断的效能。结果 NSCLC组血浆中OLFM4水平为(77.39±15.13)μg/ml,高于对照组的(14.56±6.94)μg/ml,差异有统计学意义(P〈0.05);血浆OLFM4水平与NSCLC的肿瘤大小、TNM分期、T分期及淋巴结转移有关,其中肿瘤大小〉3 cm、TNMⅢ+Ⅳ期、T_3+T_4期及有淋巴结转移者的血浆OLFM4水平均高于对应项(P〈0.05)。NSCLC患者血浆中OLFM4水平与组织中OLFM4表达呈正相关(r=0.641,P〈0.05)。血浆OLFM4水平诊断NSCLC的曲线下面积(Az)为0.862(95%CI:0.752~0.937),最佳截断值为27.11μg/ml,该截断值的灵敏度和特异度分别为81.3%和77.4%。结论 OLFM4在NSCLC患者血浆中高表达,与肿瘤大小、临床分期及淋巴结转移有关,可能在NSCLC发生发展中有一定作用;且在NSCLC早期诊断有较好价值。Objective To investigate the level of plasma OLFM4 in patients with non-small cell lung cancer( NSCLC) and its clinical significance. Methods The peripheral blood samples of 272 patients with NSCLC diagnosed in our hospital were collected and detected by enzyme-linked immunosorbent assay. The relationship between OLFM4 level and clinical pathological parameters( sex,age,tumor size,differentiation type,clinical stage,T stage,smoking history and lymph node metastasis) were analyzed. Meanwhile,the peripheral blood samples from 300 healthy subjects were selected as control. The OLFM4 levels of 156 NSCLC tissue samples were detected. The correlation between tissue and plasma levels of OLFM4 was analyzed using Person correlation analysis. Efficacy of plasma OLFM4 levels in the early diagnosis of NSCLC was evaluated using receiver operating characteristic curve( ROC). Results The OLFM4 level of NSCLC group was( 77. 39±15. 13) μg / ml,higher than( 14. 56± 6. 94) μg / ml of control group with statistical significance( P〈0. 05). The level of OLFM4 was related to the tumor size,TNM stage,T stage and lymph node metastasis of NSCLC. The patents with tumor size greater than 3 cm,Ⅲ-Ⅳ TNM stage,T3-T4 stage and lymph node metastasis had a higher plasma OLFM4 levels than their corresponding subjects( P〈0. 05). Plasma levels of OLFM4 were positively correlated with tissue( r = 0. 641,P〈 0. 05).The area under the curve of plasma OLFM4 level in the diagnosis of NSCLC was 0. 862( 95% CI: 0. 752-0. 937). The optimal cut-off value was 27. 11 μg / ml with the sensitivity and specificity rates of 81. 3% and 77. 4%,indicating a better value in the early diagnosis of NSCLC. Conclusion OLFM4 is highly expressed in the plasma of patients with NSCLC,and it is related to tumor size,clinical stage and lymph node metastasis. It may have a certain role in the occurrence and development of NSCLC,and it has good value in the early diagnosis of NSCLC.
