摘 要:目的 探讨Cullin-1在乳腺癌组织中的表达及其与临床病理学特征、预后的关系。方法 应用组织芯片技术、免疫组化EnVision法检测Cullin-1在平均随访10年的150例乳腺癌组织中表达,分析Cullin-1的表达与临床病理特征及预后的关系。结果 乳腺癌组织芯片150例点阵中有效回收标本119例,其中阳性82例,阴性37例,阳性率为68.9%。82例阳性标本中Cullin-1均为核阳性,其在ER阴性(64.3%vs71.4%)、PR阴性(65.4%vs71.6%)、HER-2阳性(63.9%vs71.1%)的患者中表达更低,但差异均无统计学意义。临床1、2、3期生存率分别为90.9%、75.6%、56.0%,晚期生存率明显低于早期(χ2=9.131,P=0.01)。Cullin-1阴性、低表达和高表达患者中位生存时间分别为117.3、134.7、38.3个月,高表达者生存期明显减少(χ2=20.8,P〈0.01)。结论 Cullin-1表达水平与乳腺癌发生、发展及预后密切相关,Cullin-1高表达是乳腺癌患者预后不良的因素,可作为临床预后评估的重要指标及作为开发靶向治疗的潜在靶标。Purpose To investigate the expression of Cullin-1 in breast carcinoma and to evaluate their correlation with the clinical pathological features and prognosis. Methods Tissue microarray technology and immunohistoehemical EnVision method were used to detect the expression of Cullin-1 in 150 cases of breast carcinoma tissue. The average follow-up time of patients was about 10 years. The relationship between Cullin-1 expression and clinical pathological features and prognosis were analyzed. Results Among 119 a- vailable cases, Cullin-1 was positive in 82 cases and negative in 37 cases. The total positive rate was 68.9%. All of the 82 positive cases were nuclear positive of Cullin-1. The expression of Cullin-1 among ER-/ER+, PR-/PR+, HER-2+/HER-2- were 64. 3% vs 71.4% , 65.4% vs 71.6% , 63.9% vs 71.1% respectively, but none of them had been significant differences. The survival rates were 90. 9% , 75.6% , 56. 0% respectively according to the clinical stage I , lI, llI. The survival rate in late stage was significantly lower than that in earlier stage ( X2 =9. 131, P =0. 01 ). The average survival time of patients with negative, low expression and high expres- sion of Cullin-1 were 117.3 months, 134. 7 months, 38.3 months, respectively. High Cullin-1 expression was correlated with reduced survival time ( X2 = 20. 8, P 〈 0. 01 ). Conclusion High Cullin-1 expression is a poor prognostic factor in patients with breast carcinoma, it can be used as an important indicator of prognosis and as a potential target to develop targeted therapy.
