摘 要:目的评价虎杖苷对大鼠神经病理性痛的影响。方法雄性SD大鼠40只,6~8周龄,体重200~230 g,采用随机数字表法分为5组(n=8):假手术组(S组)、神经病理性痛组(NP组)、虎杖苷5 mg/kg组(P1组)、虎杖苷10 mg/kg组(P2组)和虎杖苷20 mg/kg组(P3组)。NP组和P1-3组采用坐骨神经慢性压迫性损伤法制备大鼠神经病理性痛模型。S组只暴露坐骨神经,但不结扎。S组和NP组分别于术毕即刻及术后1、3、5、7 d时腹腔注射生理盐水0.1 ml;P1-3组分别于术毕即刻及术后1、3、5、7 d时腹腔注射虎杖苷5、10、20 mg/kg(溶于0.1 ml生理盐水中)。分别于术前1 d(T0)及术后1、3、5、7 d(T1-4)时测定机械缩足反应阈(MWT)和热缩足潜伏期(TWL)。于T4时痛阈测定结束后,断头处死大鼠,取L4-6节段脊髓组织,采用Western blot法测定高迁移率族蛋白1(HMGB1)、Toll样受体4(TLR4)、IL-1β、TNF-α和单核细胞趋化蛋白1(MCP-1)的表达水平。结果与S组比较,NP组T1-4时MWT降低,TWL缩短,P1组T1-4时MWT降低,T2-4时TWL缩短,P2组T1-4时MWT降低,T3,4时TWL缩短,P3组T1-4时MWT降低,NP组、P1组和P2组脊髓HMGB1、TLR4、IL-1β、TNF-α、MCP-1的表达上调(P〈0.05);与NP组比较,P2组T1-4时MWT升高,T1,2时TWL延长,P3组T1-4时MWT升高,TWL延长,P2组和P3组脊髓HMGB1、TLR4、IL-1β、TNF-α、MCP-1的表达下调(P〈0.05),P1组上述各指标差异无统计学意义(P〉0.05);与P1组比较,P2组T4时MWT升高,P3组T1-4时MWT升高,T3,4时TWL延长,脊髓HMGB1、TLR4、IL-1β、TNF-α、MCP-1的表达下调(P〈0.05);与P2组比较,P3组T3,4时MWT升高,脊髓TLR4、IL-1β、TNF-α、MCP-1的表达下调(P〈0.05)。结论虎杖苷可通过抑制脊髓炎症反应减轻大鼠神经病理性痛。Objective To evaluate the effect of polydatin on neuropathic pain in rats. Methods Forty male Sprague-Dawley rats, aged 6- 8 weeks, weighing 200-230 g, were randomly divided into 5 groups (n= 8 each) using a random number table: sham operation group ( group S) , neuropathic pain group (group NP ) , polydatin 5 mg/kg group ( group P1 ) , polydatin 10 mg/kg group ( group P2 ) , and polydatin 20 mg/kg group (group P3 ). Neuropathic pain was induced by chronic constriction injury in NP and P1-3 groups. In group S, the sciatic nerve was only exposed but not ligated. In S and NP groups, normal saline 0. 1 ml was injected intraperitoneally immediately after operation and at 1, 3, 5 and 7 days after operation (T1_4). In P1-3 groups, polydatin 5, 10 and 20 mg/kg (in normal saline 0. 1 ml) were injected intraperitoneally immediately after operation and at T1-4. At 1 day before operation (To) and T1-4, the me- chanical paw withdrawal threshold (MWT) and thermal paw withdrawal latency (TWL) were measured. After measurement of pain threshold at T4 , the rats were sacrificed, and L4_6 segments of the spinal cords were removed for determination of the expression of high-mobility group box 1 (HMGB1) , Toll-like receptor 4 (TLR4) , interleukin-lbeta (IL-1β) , tumor necrosis factor-alpha (TNF-α) and monocyte chemotactie protein-1 (MCP-1) by Western blot. Results Compared with group S, the MWT was significantly de-creased, and the TWL was significantly shortened at T1-4 in group NP, the MWT was significantly de- creased at T1-4 , and the TWL was significantly shortened at T2.4 in group Pl, the MWT was significantly de- creased at T1-4, and the TWL was significantly shortened at T3,4 in group P2, the MWT was significantly decreased at T1-4 in group Pa, and the expression of HMGB1, TLR4, IL-1β, TNF-α and MCP-1 was sig- nificantly up-regulated in NP, P1 and P2 groups (P〈0.05).
