宫颈癌患者MRI影像学表现与血清血管内皮生长因子水平的相关性研究

摘　　要：目的分析宫颈癌影像学表现与血管内皮生长因子（vascular endothelial growth factor,VEGF）相关性。方法选取我院2013年2月~2016年4月收治的宫颈癌患者46例为研究组,另外以同期来我院健康体检人员50例作为对照组,均抽取空腹静脉血,测定血清VEGF。研究组均行核磁共振检查,分析VEGF与MRI影像学检查相关性。结果观察组患者血清VEGF为（1 452.36±405.36）pg/ml,显著高于对照组的（201.36±61.85）pg/ml（P〈0.05）。宫颈癌肿瘤T1WI等信号影,T2WI略高信号,压脂序列略高信号,DWI序列高信号,病灶大小清晰。肿瘤直径≥4 cm者VEGF高于直径〈4 cm者,宫颈基层受浸润者VEGF高于未受浸润者（P〉0.05）,宫旁浸润者VEGF高于未受浸润者（P〈0.05）,阴道受损者VEGF高于未受损者（P〈0.05）,淋巴结转移者VEGF高于未出现转移者,盆壁受侵者高于未受侵者,有远处转移患者VEGF高于无转移者,差异均具有统计学意义（P〈0.05）。结论宫颈癌患者血清VEGF指标与MRI影像学表现密切相关,临床测定VEGF指标联合MRI影像学表现对肿瘤诊断有重要价值。Objective To analyze the correlation of MRI imaging and serum levels of vascular endothelial growth factor（ VEGF） in cervical cancer. Methods 46 patients with cervical cancer in our hospital from February 2013 to April 2016 were selected as research group,and 50 healthy volunteers were selected as control group. Fasting blood was extracted to determine the serum levels of VEGF in both groups. MRI exam was carried out in research group. The correlation between VEGF and MRI imaging was analyzed in the research group. Results The serum level of VEGF was（ 1 452. 36 ± 405. 36） pg / ml and（ 201. 36 ± 61. 85） pg / ml in the research group and control group respectively,there was statistic difference between two groups（ P〈 0. 05）.The MRI imaging of cervical cancer showed isointensity of T1 WI,slight hyperintense of T2 WI and fat-sequence,high signal sequences of DWI,and clearness of lesion size. The VEGF level was higher in the patients with tumor diameter more than 4 cm,or cervical grassroots invasion,or parauterine invasion,or lymph node metastasis,or vaginal invasion,or pelvic wall keen invasion,or distant metastases,and there was statistical difference in all comparisons（ P〈 0. 05）. Conclusions The serum VEGF level will increase significantly in cervical cancer patients,and is closely related to MRI imaging. The determination of VEGF combined with MRI exam can play an important role in the diagnosis of cervical cancer.
