摘 要:肿瘤微血管与胶质瘤的发生、发展、诊疗决策及预后判断密切相关。由于血脑屏障的破坏,常规增强MRI的对比强化程度不能真实反映肿瘤微血管的功能变化,临床准确诊断胶质瘤有一定困难。动态对比增强MRI是一种功能MR成像技术,不仅可以提供形态学信息,还可通过测量相关血流动力学参数对肿瘤微血管的通透性进行无创、动态、定量地评价,现已广泛应用于科研和临床工作中。综述动态对比增强MRI与胶质瘤肿瘤生物标志物表达的相关性、肿瘤分级、疗效监测以及预后评估。Tumor microvessels play a crucial role in tumor igenesis and progression, clinical decision-making and prognosis prediction. Due to the disruption of blood brain barrier, conventional MRI characterized by contrast enhancement may not truely reflect dysfunction of tumor microvessels and can hamper the accuracy of diagnosis in glioma. Dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) is a functional MRI, it provides conventional morphological information, and derives as well tracer kinetic parameters to estimate permeability of tumor microvessels noninvasively in a dynamic and quantitative manner, and hence has received considerable attention in the research and clinical field. The purpose of this article was to review the clinical application of DCE-MRI for glioma and tumor microvessels, with emphasis on the correlation analysis between DCE-MRI and glioma-associated biomarkers, glioma grading, therapy monitoring and prognosis prediction.
