非动脉炎性前部缺血性视神经病变患者与其颈动脉改变的相关性

摘　　要：目的:探讨颈动脉粥样硬化(CAA)斑块与非动脉炎性前部缺血性视神经病变的关系,为非动脉炎性前部缺血性视神经病变(NAION)患者的临床防治寻求方法。方法:选取NAION患者72例设为观察组,健康体检者81例设为对照组。两组研究对象均给予颈动脉彩色多普勒超声检查,比较两组研究对象颈动脉粥样硬化斑块检出率和颈动脉狭窄程度。结果:观察组患者的CAA斑块检出率为55.5%(40/72),显著高于对照组的32.1%(26/81),差异有统计学意义(x2=8.550,P<0.05)。观察组患者的颈动脉轻度狭窄42例,中度狭窄22例,重度狭窄8例;对照组研究对象的颈动脉轻度狭窄65例,中度狭窄14例,重度狭窄2例,两组研究对象的颈动脉狭窄程度比较,差异有统计学意义(x2=9.826,P<0.05)。结论:AION患者的发生与其CAA的存在和程度密切相关,彩色多普勒超声是监测CAA斑块最简单有效的方法,对指导NAION患者的防治有重要意义。Objective： To explore relationship between carotid artery atherosclerosis（ CAA） plaque and nonarteritic anterior ischemic optic neuropathy（ NAION）,and seek methods for clinical prevention and treatment of NAION. Methods： 72 patients with NAION were regarded as observation group,and 81 cases of health examination were regarded as control group. The two groups were given the carotid artery color Doppler ultrasound examination. The carotid atherosclerotic plaque detection rate and the degree of carotid stenosis of the two groups were compared. Results： The detection rate of CAA plaque was 55. 5%（ 40 /72） in observation group,which was higher than that in control group [32. 1%（ 26 /81） ],and there was a significant difference（ x2= 8. 550,P0. 05）. In the observation group,mild stenosis,moderate stenosis and severe stenosis occurred in 42 cases,22 cases and 8 cases. Separately,while those in control group were 65 cases,14 cases and 2 cases,separately. There was the significant difference in stenosis degree of carotid arteries between the two groups（ x2= 9. 826,P〈0. 05）. Conclusions： The occurrence of nonarteritic AION is closely related to the existence and degree of CAA. Color Doppler ultrasonography is the most simple and effective method to monitor CAA plaque,which has a clinical significance in the prevention and treatment of nonarteritic anterior ischemic optic neuropathy.
