摘　　要：目的应用高分辨率食管测压系统(HRM)探讨咽异感症患者食管上括约肌(upper esophageal sphincter,UES)的功能特征。方法选取自2013年5月-2015年5月就诊于云南省第一人民医院的62例咽异感症患者,明确排除口腔、鼻腔、咽喉、颈部及食管器质性疾病。48例正常志愿者作为对照。经鼻腔导入HRM测压导管,观察静息及吞咽时食管压力变化,主要观察指标UES静息压平均值、UES残余压平均值、UES松弛率。结果 62例咽异感症患者与正常对照组相比,UES静息压、UES残余压降低具统计学意义(P<0.05),UES松弛率无明显变化(P>0.05)。结论观察组较对照组UES静息压、UES残余压的降低有统计学意义,咽异感发病可能与食管上括约肌压力降低有关,表明咽异感症可能与胃食管反流有关。对不明原因咽异感患者诊治具有一定的参考价值。Objective To investigate the upper esophageal sphincter function in patients with pharyngeal paraesthesia by esophageal high-resolution manometry. Methods Taking the 62 patients with pharyngeal paraesthesia, who were treated from May 2013 to May 2015 in The First People＇s Hospital of Yunnan Province,patients with oral, nasal, throat, neck and esophageal organic disease were exclusive. 48 normal volunteers as control.HRM pressure measuring catheter were through nasal cavity.The changes of esophageal pressure during rest and swallowing were observed,The mean value of UES resting pressure,mean value of UES residual pressure and UES relaxation rate were mainly observed. Results 62 patients with pharyngeal paraesthesia compared with normal control group, UES resting pressure and UES residual pressure were less, and statistically significantly（P〈0.05）,UES relaxation rate had no significant differenve（P〉0.05）. Conclusion Experimental group compared with control group,UES resting pressure and UES residual pressurere are less,and statistically significantly. The incidence of pharyngeal sensation may be related to the lower upper esophageal sphincter pressure, and suggests that pharyngeal paraesthesia may be related to gastroesophageal reflux. The diagnosis and treatment of patients with pharyngeal paraesthesiahas may have certain value.
