摘 要:目的探讨富氢盐液对大鼠脑缺血-再灌注(I-R)损伤后缺血半暗带(IP)区诱导型一氧化氮合酶(iNOS)表达和细胞凋亡的影响。方法雄性SD大鼠54只随机均分为假手术组(S组)、I-R组和富氢盐液组(H组)。以栓线法阻断大鼠大脑中动脉120min制备I-R损伤模型。H组于再灌注即刻腹腔注射富氢盐液5ml/kg(0.6mmol/L),其余两组腹腔注射等容量生理盐水。再灌注48h后行神经功能缺陷评分;之后迅速断头取脑,测定梗死核心区和IP区iNOS蛋白表达、iNOS酶活性和一氧化氮(NO)含量,观察细胞凋亡情况。结果与I-R组比较,H组脑梗死面积缩小,神经功能缺陷评分获不同程度改善(P<0.05)。缺血48h后,I-R组梗死核心区和IP区均检测到iNOS活性升高和免疫阳性反应,且IP区iNOS免疫阳性反应强于梗死核心区(P<0.05)。与I-R组比较,H组能够抑制梗死核心区和IP区iNOS酶活性和NO产生,下调IP区iNOS蛋白表达,IP区TUNEL阳性细胞数目降低(P<0.05)。结论富氢盐液可能通过下调IP区iNOS蛋白表达以及降低iNOS酶活性和NO含量,减少细胞凋亡,从而发挥脑保护作用。Objective To explore the influence of hydrogen-rich saline on focal cerebral ischemia-reperfusion(I-R)injury-induced nitric oxide synthase(iNOS)expression and apoptosis in the ischemic penumbra(IP)in rats.Methods Fifty-four adult male SD rats were equally randomized into 3groups of S(sham operation),I-R(ischemia for 120min)and H(treated with hydrogen-rich saline 5ml/kg intraperitoneally immediately after reperfusion).At 48 hours after cerebral ischemia,neurological function was scored,after which the animals were decapitated and the brain was immediately removed for microscopic examination and determination of iNOS protein expression,NO content and apoptosis in the ischemic core(IC)and IP.Results Compared to group I-R,the cerebral infarct size was smaller and neurological score was improved in group H(P0.05).The iNOS activity was increased and iNOS immunoreactivity was positive in both IP and IC areas,which was stronger in IP area than that in IC area(P0.05).Compared to group I-R,the iNOS activity and NO production in both areas were less in group H(P0.05).So did the iNOS expression and TUNEL positive cell numbers in IP area in group H(P0.05).Conclusion Hydrogen-rich saline has protective effect against cerebral I-R injury and reducing apoptosis by inhibiting iNOS protein expression and suppressing iNOS activity and NO content in IP area.
