摘　　要：目的比较应用阿加曲班与阿加曲班联合氯吡格雷治疗急性脑梗死(ACI)的效果及安全性。方法 ACI患者84例随机分为单药组和联合组。两组患者常规应用改善循环、脑保护、控制血压血糖、调脂治疗,以及必要的脱水、抗炎、保护胃黏膜等治疗。单药组在常规药物治疗基础上应用阿加曲班治疗,联合组为常规治疗基础上应用阿加曲班联合氯吡格雷治疗。治疗前和治疗1周、2周后以美国国立卫生院卒中(NIHSS)量表评分评估两组患者神经功能变化情况同时观察记录患者的不良反应。结果 1单药组及联合组ACI患者治疗1周、2周后较治疗前NIHSS评分均降低,差异有统计学意义;2治疗1周、2周后,NIHSS评分联合组低于单药组,差异有统计学意义;3两组不良反应对比无明显差异。结论 ACI的患者在常规治疗基础上,阿加曲班联合氯吡格雷治疗效果优于单独选择阿加曲班治疗,且不增加不良反应。Objective To compare the efficacy and safety of argatroban versus argatroban plus clopi- dogrel in the treatment of acute cerebral infarction （ACI）Methods 84 patients with ACI who routinely re- ceived circulation improvement, cerebra protection, blood pressure and glucose contral, blood lipid regulation and selectively received treatment of dehydration, anti-inflammatory, gastric mucosa protection and so on were randomly divided into two groups：single treatment group （argatroban was used according to the instructions） and combinational treatment gourp （argatroban plus clopidogrel were used according to the instructions）. Ac- cording to NIHSS, We evaluated neurological function changes in two groups before treatment, 1 week after treatment and 2 weeks after treatment then observed and recorded the adverse reactions of these pa- tients. Results ① In both groups, N1HSS was reduced after one week treatment and two weeks treatment, compared with that before treatment. The results showed statistically significant difference in each group. ②NIHSS of combinational treatment group was less than that of single treatment group,, which was statistically significant difference. ③ There was no significant difference of adverse reactions between two groups. Conclusion The treatment effect of argatroban plus clopidogrel was superior than it of argatroban in patients with ACI who have received conventional treatment.
