中药联合腹腔镜治疗粘连性肠梗阻36例

摘　　要：目的:观察茴香枳术汤联合腹腔镜治疗粘连性肠梗阻的临床疗效。方法:将72例患者随机分为观察组、对照组各36例。2组均静脉全麻,由同组医师在腹腔镜下完成手术,观察组在术后第1天起即加用茴香枳术汤,每日1剂,分2次胃管注入,每次100 m L,直到腹胀消失,大便通畅为止。观察2组临床疗效及肛门排气时间、症状消失时间、WBC恢复正常时间、气液平消失时间。结果:治愈率、总有效率观察组分别为66.67%、91.67%,对照组分别为50.00%、77.78%。临床疗效观察组优于对照组,差异有统计学意义(P<0.05)。肛门排气时间、症状消失时间、WBC恢复正常时间、气液平消失时间观察组均少于对照组,差异有统计学意义(P<0.05)。结论:茴香枳术汤联合腹腔镜治疗粘连性肠梗阻临床疗效显著,且术后康复快、并发症少及住院时间短。Objective： To observe clinical effects of Hui Xiang Zhi Zhu Tang combined with laparoscope in treating adhesive ileus. Methods： All 72 patients were allocated to the observation group and the control group randomly,36 cases each group. All the patients were performed the surgery under the laparoscope after intravenous general anesthesia by the physician in the same team, the observation group took Hui Xiang Zhi Zhu Tang since the first day after the operation, one dose per day, injecting by stomach tube, twice, 100 m L each time, till abdominal distension disappeared and bowels opened. Curative effects, anal exhaust time, symptom disappearing time, the time of WBC restoring to normal, gas-fluid levels disappearing time were observed in both groups. Results： Cure rate and total effective rate of the observation group were 66.67% and 91.67% respectively, higher than 50.00% and 77.78% of the control group. The observation group was superior to the control group in clinical effects（P〈0.05）. The observation group was less than the control group in anal exhaust time, symptom disappearing time, the time of WBC restoring to normal and gas-fluid levels disappearing time（P〈0.05）. Conclusion： Hui Xiang Zhi Zhu Tang combined with laparoscope is effective in treating adhesive ileus, and the therapy shows the advantages of rapid rehabilitation after the operation, less complications and short hospitalization time.
