摘　　要：目的探讨胰腺实性-假乳头状瘤(SPTP)的CT表现,并总结多层螺旋CT(MSCT)的应用价值。方法对22例SPTP患者进行MSCT检查,对其影像学特征进行分析。结果 22例病变均为单发,体尾部交界处多见为12例;形态上以卵圆形为主为13例;肿瘤直径≤3 cm者8例,>3 cm者14例;病灶边缘光滑、边界清楚者17例,边界不清或无明显边界者5例;15例显示为清晰完整的包膜,增强扫描有强化,7例未见明确包膜显示;囊性为主型6例,实性为主型7例,囊实相间型9例;12例表现为密度均匀、10例表现为密度混杂,其中有出血者2例,钙化者6例。增强扫描实性部分呈渐进性延迟强化,囊性部分不强化,可伴有钙化、出血等,所有病例均未见远处转移。结论 MSCT对SPTP病变诊断准确,有较高的诊断价值。Objective To investigate CT manifestation of solid pseudopapillary tumor of pancreas（SPTP）, and to summarize application value of multi-slice spiral CT（MSCT）. Methods A total of 22 SPTP patients received MSCT examination, and their imaging characteristics were analyzed. Results All the 22 lesions were single, with common junction of the body and tail in 12 cases; main rounded and oval form in13 cases; diameter of tumor ≤ 3 cm in 8 cases, 3 cm in 14 cases; smooth margin and clear boundary in 17 cases, obscure or no visible boundary in 5 cases; clear and complete diolame in 15 cases strengthening in enhancement scan, unclear diolame in 7 cases; main cystic type in 6 cases, solid type in 7 cases, cystic and solid type in 9 cases; uniform density in 12 cases, mixed density in 10 cases, hemorrhage in 2 cases and calcification in 6 cases. Enhancement scan of solid part showed progressive delayed reinforcement, without cystic reinforcement, along with calcification and hemorrhage. There was no distant metastasis in any case. Conclusion MSCT shows accurate diagnosis for SPTP, and it contains highly diagnostic value.
