摘 要:目的探讨自身免疫性肝炎(autoimmune hepatitis,AIH)Ⅰ型患者自身抗体与其血清生物化学指标的相关性。方法选取2010年12月至2014年12月于湖北省中医院就诊的163例AIH-Ⅰ型患者为研究组,另外纳入197例健康献血者为对照组,采用酶联免疫法(ELSIA)检测AIH-Ⅰ型患者的抗核抗体(antinuclear antibody,ANA)、抗纤维肌动蛋白抗体(anti-filamentous actin antibody,AFA)和抗平滑肌抗体(anti-smooth muscle antibody,ASMA),采集全部研究对象的血清进行生物化学指标(ALT、AST、ALP、TBil、γ-GT、GLO)和免疫球蛋白(IgG、IgA、IgM)的检测,应用SPSS l9.0统计软件对检测结果进行统计学分析。结果 AIH-Ⅰ型患者ANA、AFA和ASMA的阳性率分别为84.66%、75.46%和70.55%。与对照组相比,AIH-Ⅰ型患者生物化学指标除ALP外,其他均显著升高。AIH-Ⅰ型患者的ANA与AST、TBil低度相关,AFA与ALT、AST和IgG低度相关,ASMA与ALT、AST和Ig M低度相关。结论 AIH-Ⅰ型患者自身抗体ANA、AFA、ASMA与其血清生物化学指标的相关性较差,二者对AIH-Ⅰ的诊断价值不能相互替代。Objective To investigate the correlation of autoantibodies and biochemical indicators in patients with autoimmune hepatitis(AIH) type Ⅰ. Methods Total of 163 patients with AIH-Ⅰ from December 2010 to December 2014 in Hubei Provincial Hospital of Traditional Chinese Medicine were selected as the research group and other 197 blood donors were selected as the controls. The antinuclear antibody(ANA), anti-filamentous actin antibodies(AFA) and anti-smooth muscle antibodies(SMA) in patients with AIH-Ⅰwere evaluated by ELISA and the serum biochemical indicators in patients with AIH-Ⅰ type and the controls were detected, all the results were analyzed by SPSS 19.00 software. Results The positive rates of ANA, AFA and ASMA in patients with AIH-Ⅰ type were 84.66%, 75.46% and 70.55%, respectively. Except for ALP, the biochemical indicators were significantly higher in patients with AIH-Ⅰ than those of the controls. The ANA was lowly correlated with AST and TBil, the AFA was lowly correlated with ALT, AST and Ig G and the ASMA was lowly correlated with ALT, AST and IgM. Conclusions The autoantibodies including ANA, AFA and ASMA have a low correlation with biochemical indicators in patients with AIH-Ⅰ, and the diagnostic value of autoantibodies and biochemical indicators can not be replaced by each other.
