摘 要:目的明确趋化因子受体CXCR4在非小细胞肺癌(NSCLC)组织的表达及其与TNM分期和纵隔淋巴结转移的关系,探索CXCR4作为肺癌患者预后指标的可行性。方法收集医院标本库中的肺癌标本(NSCLC组)61例,全部病人术前均未经过化疗或放疗,登记病人性别、年龄、组织学类型、TNM分期、是否淋巴结转移等临床资料,采用免疫组织化学(IHC)技术检测CXCR4的表达阳性率。对照组63例为正常肺组织(取自距肿瘤边缘5 cm以上处,经病理检查确证为不含有肿瘤细胞的正常肺组织)。结果NSCLC组中CXCR4阳性表达率为52.5%(32/61),对照组中CXCR4阳性表达率为17.5%(11/63),差异有统计学意义(χ^2=16.759,P〈0.001)。CXCR4表达与肺癌TNM分期、纵隔淋巴结转移明显相关(P〈0.05),而与肿瘤组织学类型、性别、年龄无关(P〉0.05)。结论 CXCR4在NSCLC组织中高表达,与淋巴结转移、TNM分期紧密相关,在NSCLC生长、侵袭和转移中有着十分重要的作用。Objective To investigate the expression of chemokine receptor CXCR4 in non-small cell lung cancer(NSCLC)tissues and its relationship with TNM stage and metastasis of mediastinal lymph nodes, and explore the feasibility of using CXCR4 expression as a prognostic factor for patients with NSCLC. Methods The lung cancer specimens(NSCLC group)from 61 patients who were without radiotherapy or chemotherapy before resection were collected from the tissue bank of our hospital, and the gender, age, histological type, TNM stage and lymph node metastasis were recorded. Immunohistochemical(IHC) staining was used to detect the expression of CXCR4. As the control group, the normal lung tissues from 63 patients were obtained from the sites beyond 5 cm away from the borderline of tumor tissue,which were indicated by pathology.Results The expression rate of CXCR4 among the 61 NSCLC specimens was 52.5%(32 / 61), however, the rate for control group was only 17.5%(11 / 63); the difference was statistically significant(χ^2=16.759,P〈0.001). CXCR4 expression was associated with TNM stage and lymph node metastasis, but not with age, gender and histological type. Conclusion CXCR4 is highly expressed in non small cell lung cancer tissues, which is closely related to lymph node metastasis and TNM stage,and may play an important role in the growth, invasion and metastasis of NSCLC cells.
