不同程度干眼症患者视觉质量参数变化及临床意义

摘　　要：目的探讨不同程度干眼症患者视觉质量参数变化及视觉质量的影响。方法将2013年4月至2015年4月在华北石油管理局总医院眼科就诊的干眼症患者100例按照病情分为轻度组43例、中度组37例和重度组20例,另选择同期体检的健康人群40例作为健康对照组;各组受试对象均采用基于双通道技术的视觉质量分析系统检测自然状态下斯特列尔比(SR)、调制传递函数截止频率(MTF cut off)、客观散射指数(OSI),检测不眨眼过程中OSI动态变化水平,并记录OSI动态变化均值(OSI均值)。结果四组受试对象球镜度数、柱镜度数比较差异无统计学意义(P>0.05),轻度组、中度组、重度组干眼症患者BUT、SchirmerⅠtest检查结果均显著低于健康对照组[(5.10±0.09)s,(5.07±0.09)s,(5.00±0.07)s比(12.21±1.03)s,(10.04±0.10)mm/5 min,(9.95±0.07)mm/5 min,(9.93±0.10)mm/5 min比(17.83±1.01)mm/5 min](P<0.05),而轻度组、中度组、重度组三组间BUT、SchirmerⅠtest结果差异无统计学意义(P>0.05)。中度组、重度组SR、MTF cut off显著低于轻度组和健康对照组[(0.17±0.02),(0.13±0.01)比(0.24±0.03),(0.25±0.05);(30.73±3.41)c/deg,(27.69±2.09)c/deg比(39.86±3.75)c/deg,(40.53±4.71)c/deg],轻度组、中度组、重度组OSI显著高于健康对照组[(0.62±0.07),(0.83±0.10),(0.91±0.11)比(0.43±0.05)](P<0.05);轻度组与健康对照组SR、MTF cut off比较差异无统计学意义(P>0.05);中度组、重度组OSI显著高于轻度组(P<0.05);重度组SR、MTF cut off又显著低于中度组,OSI显著高于中度组(P<0.05)。轻度组、中度组、重度组OSI均值显著高于健康对照组[(0.87±0.07),(1.84±0.14),(2.13±0.27)比(0.61±0.04)],中度组、重度组OSI显著高于轻度组,重度组OSI显著高于中度组,差异有统计学意义(P<0.05)。干眼症患者自然状态下OSI与SR、MTF cut off均呈负相关(r=-0.879,-0.709,P<0.05),与年龄无明显相关性(r=-0.236,P>0.05),干眼症患者膜动态状态下OSI均值与�Objective To explore the visual quality parameters changes of different degrees of xerophthalmia and the influence on the visual quality. Methods A total of 100 xerophthalmia patients treated in Huabei Oil Field Hospital from Apr. 2013 to Apr. 2015 were divided into a mild group of 43 cases,a moderate group of 37 cases and a severe group of 20 cases according to the disease condition, and another 40 cases of healthy people for heath examination were included as a control group. All subjects received visual quality analysis system by dual channel technology to detect the Strehl ratio （SR） ,the cut-off frequency of modulation transfer function （ MTF cut off）, the objective scatter index （OSI） under natural state, and the OSI dynamic changes without blinking,and the mean value of the dynamic changes of OSI（ OSI means） were recorded. Results The spherical degree, cylinder degree of the four groups had no significant differences （P 〉 0. 05 ） ,the BUT, Schirmer I test examination results of the mild group, moderate groupand severe group were significantly lower than the control group[ （5. 10±0.09） s,（5.07±0.09） s,（5.00±0.07） s vs （12.21 ±1.03） s,（10.04 ±0. 10） mm/5 win, （9.95 ± 0. 07 ） mm/5 win, （9.93 ± 0. 10） mm/5 win vs （ 17. 83 ± 1.01 ） mm/5 mini （P 〈 0. 05 ）, and there were no statistically significant differences between the mild group, moderate group and severe group （ P 〉 0. 05 ）.
