多层螺旋CT对原发性十二指肠腺癌的诊断价值

摘　　要：目的探讨多层螺旋CT对原发性十二指肠腺癌的诊断价值。方法回顾性分析10例内镜活检证实或手术病理证实的原发性十二指肠腺癌的临床资料及CT表现。结果:10例十二指肠腺癌中,6例为肿块型,4例为缩窄型,表现为腔内息肉状、菜花状软组织肿块或肠壁不规则或环形增厚,伴肠腔狭窄4例,近段肠管扩张2例,多数边界清楚,呈中度或重度均匀、不均匀强化;邻近组织浸润4例,淋巴结肿大3例,远处转移3例。结论多层螺旋CT对十二指肠腺癌具有较好的诊断价值,能清晰显示其形态、大小,判定有无周围浸润、淋巴结及远处转移,对病变的发现以及降低误诊、漏诊率大有帮助。Objective： To explore the value of multislice CT in the diagnosis of primary duodenal adenocarcinoma.Methods： Retrospective analysis was carried out on clinical data and CT features in 10 patients confirmed primary duodenal adenocarcinoma by endoscopic biopsy or surgical pathology.Results： Among 10 patients with duodenal adenocarcinoma, of which 6 mass lesions, 4 intestinal narrowing presenting with polypoidal, cauliflower soft tissue or irregular intestinal wall or round lesion thickening, 4 lumen stenosis, 2 dilated proximal intestinal canal with clear margins, or moderate or severe homogeneous and heterogeneous enhancement, 4 adjacent tissues infiltration, 3 lymph node enlargement, 3 distant metastasis.Conclusions： Multislice CT is valuable in diagnosing duodenal adenocarcinoma, which can greatly help for improving detection, decreasing misdiagnosis and missed diagnosis. The morphology and the size can be clearly seen, to determine with or without infiltration to circumferential tissue, distance metastases and lymph node.
