摘  要:目的:探讨眼外伤患者治疗后血清Th亚群细胞因子变化的临床意义。方法眼外伤30例(30眼)为研究对象。根据受伤原因及临床表现将患者分为治疗一组(无眼球穿孔的眼外伤组)和治疗二组(眼球穿孔伤组)各15例(15眼),并设立健康人对照组15例(15眼)。采用ELISA法检测血清细胞因子IL-2、IFN-γ、IL-10、IL-17含量的动态变化。结果治疗一组受伤早期较对照组IL-2升高明显(P<0.05),IFN-γ及IL-17降低明显(P<0.05);治疗二组较对照组除 IFN-γ、IL-17降低外,其余均升高(P<0.05)。治疗后治疗一组各种细胞因子均较治疗前降低(P<0.05),治疗二组治疗后除IL-17外其它细胞因子均降低( P<0.05)。结论眼外伤患者治疗后血清中Th亚群细胞因子的动态变化与病程进展有密切联系,临床上可作为除血象检查外的另一个评定抗感染治疗是否有效的监测指标。经抗感染治疗后可降低Th1、Th17炎性因子的大量分泌和白细胞、中性粒细胞的增生,有利于降低自身免疫性交感性眼炎的发生。Objective To investigate the clinical significance of the changes of Th subgroup cytokines in ocular traumatic patients .Methods The study objects include 30 eyes of 30 cases with ocular trauma.The patients were divided into group 1 ( nonperforating eye injury ) and group 2 ( perforating eye injury) with 15 eyes of 15 cases in each group .And 15 healthy persons were enrolled as the control group . The dynamic changes of the concentration of serum cytokines IL-2, IFN-γ,IL-10,IL-17 were detected by ELISA.The results were compared among the three groups .Results In group 1, the concentration of IL-2 was higher than that in control group at early injury period , the difference was statistically significant ( P〈0.05);and the concentration of IFN-γand IL-17 were lower than the control group , the difference was statistically significant (P〈0.05).In group 2, IFN-γand IL-17 decreased comparing with the control group, but the IL-2 and IL-10 increased (P〈0.05).After treatment, all 5 cytokines in group 1 were decreased (P〈0.05).Except for IL-17, the rest cytokines in group 2 decreased(P〈0.05).Conclusion The dynamic changes of Th subgroup cytokines in ocular traumatic patients is closely related to disease progression.It can be an alterative inspection item for checking the validate of the anti-infection treatment. After anti-infection treatment, the secretion of Th1,Th17 decreased, the proliferation of white blood cells droped, which is benefit for reducing the risk of sympathetic ophthalmia .
