青光眼小梁切除联合羊膜移植术的临床效果

摘　　要：目的：观察青光眼小梁切除联合羊膜移植术的临床效果。方法2013年6月至2014年6月行滤过手术的青光眼共62例（62眼）。按手术方式的不同，将接受单纯小梁切除术者作为对照组，将接受小梁切除联合羊膜移植术者作为观察组，每组各31例（31眼）。术后随访12个月，统计对比两组降眼压效果、滤过泡形成情况及并发症发生率。结果观察组平均眼压为（14．02±1．67） mmHg（1 mmHg＝0．133 kPa），低于对照组的（16．57±3．28）mmHg，差异有统计学意义（t＝3．8576， P＝0．0020）；观察组手术成功率为96．77％（30／31），高于对照组的83．87％（26／31），差异有统计学意义（χ2＝9．5168，P＝0．0020）；观察组Ⅰ型＋Ⅱ型功能性滤过泡形成率较对照组高，差异有统计学意义（χ2＝13．8529，P＝0．0001）；两组患者均无严重并发症发生。结论小梁切除联合羊膜移植术治疗青光眼可达到良好的降眼压效果。Objective To observe the clinical effect of the combined trabeculectomy and amniotic membrane transplantation in the treatment of glaucoma .Methods Sixty-two eyes of 62 glaucoma patients treated with filtration surgery in our hospital from June 2013 to June 2014 were enrolled in the study .The patients treated with trabeculectomy were in the control group of 31 cases, and who treated with trabeculectomy and amniotic membrane transplantation were in observation group of 31 cases. The postoperative intraocular pressure , bleb formation and complications were analyzed .The follow-up was 12 months.Results Compared with control group （16.57 ±3.28） mmHg, the average intraocular pressure in observation group （14.02 ±1.67） mmHg （1 mmHg =0.133 kPa） was lower, the difference was statistically significant（t=3.8576, P=0.0020）; the observation group success rate was 96.77%（30/31）, which was higher than that of the control group（83.87%, 26/31）, the difference was statistically significant（x2 =9.5168,P =0.0020）.The type I and II functional bleb of the observation group were higher than that of the control group, the difference was statistically significant （ x2 =13.8529, P =0 .0001 ） .There was no serious complication in the two groups .Conclusion The combined treatment of trabeculectomy and amniotic membrane transplantation has good effect in reducing intraocular pressure for glaucoma patients .
