经外周静脉超声造影在宫腔占位病变诊断中的初步应用

摘　　要：目的:通过观察宫腔占位病变的超声造影特征,分析良、恶性占位时间-强度曲线量化指标的差异,为临床早期发现内膜恶性病变提供更准确的参考依据。方法:分别对78例宫腔占位(良性46例,恶性32例)进行外周静脉超声造影检查,分析宫腔内病灶灌注增强方式;使用Qlab脱机分析软件,获得时间-强度曲线(TIC),比较良性组与恶性组间上升时间、达峰时间、峰值强度、半降时间及内洗斜率等指标,对病灶的良恶性提供初步诊断。结果:宫腔内病灶造影灌注的方式有三种:整体快速增强、周边向中央汇聚、中央向外周灌注,第一种方式在恶性组中多见(28/32,87.5%),后两种多见于良性病灶(39/46,84.8%);TIC量化指标中上升时间、达峰时间、半降时间及内洗斜率在两组中具有显著统计学差异,峰值强度在组间无统计学差异。结论:通过超声造影成像技术及量化指标分析能初步鉴别宫腔占位性病变良恶性,为临床提供更多诊断信息。Objective：To evaluate the clinical value of contrast-enhanced （CEUS） for detecting the occu- pation diseases in uterine cavity, and to analyze the differences of the time intensity curve （TIC） and improve the diagnostic rate of ultrasonography. Methods. Seventy-eight patients with occupa- tion lesions in the uterine cavity underwent contrast-enhanced ultrasound with low MI, including 32 cases with endometrial carcinoma, 26 cases with submucous myomas, 12 cases with endome- trial polyp, 5 cases with endometrial hyperplasia and 3 cases with uterine hydrops. The perfusion characteristics of benign and malignant lesions of the uterine cavity were observed. By Qlab off- line software, the time intensity curve was drawn of region of interest uterine cavity lesions, and the contrast parameters were acquired as the arrival time, to-peak time, peak intensity, half-wash time, and wash-out slope. Results： There are three perfusion patterns in the early phase： homo- geneous or inhomogeneous high enhanced, exterior to interior and interior to exterior perfusion. The difference of the arrival time, to-peak time, half-wash time and wash-out slope between the two groups was statistically significant. However, no difference was found in peak intensity.Conclusion： CEUS may have potential clinical value for detecting occupation diseases in the uter- ine cavity.
