朊蛋白在喉鳞状细胞癌中的表达及临床意义

摘　　要：目的:观察朊蛋白(PrPc)在喉鳞状细胞癌(简称鳞癌)中的表达情况,探讨PrPc与喉鳞癌临床病理特征的关系。方法:选取2000年6月至2013年4月收入院的喉鳞状细胞癌患者癌组织60例,喉鳞状上皮不典型增生患者病灶组织20例,非肿瘤患者的喉部正常粘膜组织20例。运用免疫组化(Envision)法检测PrPc在各组织中的表达水平,分析其与喉鳞癌患者的临床病理特征如:性别、年龄、临床分期、分型、病理分化程度、有无淋巴结转移的关系。结果:PrPc在喉鳞癌组织,喉不典型增生组织及正常粘膜组织中的表达阳性率分别为:55%(33/60),25%(5/20)及5%(1/20)。PrPc在3种组织中表达差异有统计学意义(<0.05)。并且PrPc在喉鳞癌中的表达阳性率随其病理分化程度的降低而升高(P<0.05)。PrPc在Ⅲ+Ⅳ期肿瘤组中的阳性表达率明显高于Ⅰ+Ⅱ期病例组(<0.05)。结论:PrPc的高表达与喉鳞癌的发生发展有密切关系,能为喉鳞癌的早期诊断和预后评估提供线索。Objective： To observe the expression of prion protein（PrPc） in laryngeal squamous cell carcinomas（abbreviated as squamous cell carcinomas） and to explore the relationship between PrPcexpression and clinical pathologic features of laryngeal squamous cell carcinoma.Methods：Application of Immunohistochemistry（Envision）assay to detect the expression situation of PrPcin 60 cases of laryngeal squamous cell carcinoma,in 20 cases of laryngeal squamous atypical hyperplasia tissue and in 20 cases normal laryngeal mucosa taken from patients without tumor.in order to analyze the relationship between the expression level of PrPcand clinical pathological features of patients with laryngeal squamous cell carcinoma,such as,gender,age,clinical stages and pathological cell differentiation,and with or without lymph node metastasis.Results：The positive expression rate of PrPcin laryngeal squamous cell carcinomas organization,laryngeal squamous atypical hyperplasia tissue and normal laryngeal mucosa was 55%（33/60）,25%（5/20） and 5%（1/20） respectively.There were statistically significant differences in expression of3 kinds of tissues（P〈0.05）.With the decrease of pathological differentiation degree,the expression of PrPcin laryngeal squamous cell carcinomas organization rises（P〈0.05）.The positive expression rate of PrPcin laryngeal squamous cell carcinoma tissues of the advanced stage patients group（III stage and IV stage） was significantly higher than that of laryngeal squamous cell carcinoma tissues of I and II stage patients group（P〈0.05）.Conclusions：The high expression of PrPcwas closely associated with the genesis and development of laryngeal carcinomas.
