摘 要:目的探讨25-羟基维生素D水平与中国人群乳腺癌的关系,为乳腺癌的防治和病因探索提供证据。方法计算机检索中国期刊全文数据库、中文科技期刊全文数据库、万方数据库、Pubmed等大型数据库中从建库至2015年7月发表的有关25-羟基维生素D水平与中国人群乳腺癌关系的前瞻性研究文献,同时检索可纳入文献的参考文献。采用Meta分析方法,应用RevMan 5.3软件评价25-羟基维生素D水平与中国人群乳腺癌的关系。结果 125-羟基维生素D水平在中国人群乳腺良恶性肿瘤中比较差异有统计学意义,SMD=-1.81(95%CI:-2.65^-0.96)。2血清钙水平在中国人群乳腺良恶性肿瘤中比较差异有统计学意义,SMD=-0.32(95%CI:-0.52^-0.13)。3中国人群乳腺癌亚组分析显示,外周血25-羟基维生素D水平在乳腺癌早期与乳腺癌晚期间比较差异有统计学意义,SMD=0.93(95%CI:0.32~1.54)。外周血25-羟基维生素D水平在乳腺癌有无淋巴结转移间比较差异有统计学意义,SMD=-1.82(95%CI:-2.24^-1.39)。结论外周血25-羟基维生素D水平与中国人群乳腺癌存在着密切的联系,并可作为预测乳腺癌转移的重要参考指标。补充维生素D对中国人群乳腺癌的预防有一定意义。Objective To investigate the relationship between 25-hydroxy-vitamin-D and breast cancer among Chi- nese population, so as to provide evidence for the prevention strategy and pathogenesis exploration of breast cancer. Methods Computer-based retrieval was made from the large databases such as Chinese Journal Full-text Database, Chinese Sci-tech Periodical Full-text Database, Wanfang Database and Pubmed to get the prospective research litera- tures about the relationship between 25-hydroxy-vitamin-D and breast cancer among Chinese population published from the database foundation to July 2015.The references that could be included into literatures were also retrieved. Meta-analysis method and RevMan 5.3 sotlware were adopted to evaluate the relationship between 25-hydroxy-vita- min-D level and breast cancer among Chinese population.Results (1)25-hydroxy-vitamin-D level showed differences of statistical significance in benign and malignant mammary tumor among Chinese population SMD= - 1.81 (95% CI: --2.65 - --0.96).(2)Serum calcium level showed differences of statistical significance in benign and malignant mam- mary tumor among Chinese population SMD= - 0.32 (95 % CI: - 0.52 - -- 0.13). (3) Subgroup analysis of breast cancer among Chinese population showed 25-hydroxy-vitamin-D level in peripheral blood showed differences of statistical sig- nificance in the early and late stage SMD = 0.93 (95 % CI: O. 32 - 1.54). 25-hydroxy-vitamin-D level in peripheral blood showed differences of statistical significance in breast cancer with or without lymphatic metastasis.Conclusion 25-hy- droxy-vitamin-D level in peripheral blood is closely related to breast cancer among Chinese population, which can be taken as an important reference index to predict breast cancer metastasis.Vitamin D supplementation can help prevent breast cancer among Chinese population.
