玻璃体切割术结合ICL植入治疗严重眼外伤并晶状体脱位

摘　　要：目的观察玻璃体切割结合人工晶状体植入术治疗严重眼外伤并晶状体脱位,不同晶状体使用的临床效果,探讨严重眼外伤并晶状体脱位的治疗。方法选取严重眼外伤并晶状体脱位患者80例,随机分为两组各40例。两均采用相同方法进行玻璃体切割手术及脱位晶状体取出术,A组使用爱尔康CZ70BD一片式PMMA悬吊式人工晶状体植入,B组使用博士伦AKreos Adapt四襻固定亲水性丙烯酸折叠式晶状体植入。观察比较两组术后最佳矫正视力及并发症发生情况。结果经过治疗两组视力均较治疗前提高(P<0.05),但两组之间最佳矫正视力差别不大(P>0.05)。A组术后出现出血、继发青光眼、晶状体偏位等并发症的频率分别为17.50%、15.00%和0%,B组分别为15.00%、17.50%和0%,两组相比差别不大(P>0.05)。结论在掌握手术适应证的情况下,采用玻璃体切割术结合人工晶状体一期植入术治疗严重眼外伤并晶状体脱位切实可行,两种晶状体各有优缺点,应根据患者病情进行选择。Objective To observe the clinical effects of vitrectomy and ICL （intraocular lens） implantation on serious ocular injury with lens dislocation by applying different lens. Methods A total of 80 patients with serious ocular injury with lens dislocation were selected and randomly divided into two groups （n=40, respectively）. The two groups were treated with vitrectomy and removal of dislocated lens, and then, group A was implanted with Alcon CZ7OBD one-piece PMMA suspensory ICL and group B with Bausch and Lomb AKreos Adapt four-loop fixed hydrophilic acrylic folding lens. The post-operation best corrected visual acuity （BCVA） and complications in the two groups were compared, Results The vision in the two groups was improved after the treatment（P 〈 0.05）. However, there was no great difference between the BCVAs in the two groups （P 〉 0.05）. The incidence of post-operation bleeding, secondary glaucoma, deviation of lens and other complications was 17.50%, 15.00% and 0% in group A and 15.00%, 17.50% and 0% in group B, without great difference （P 〉 0.05）. Conclusion On the premise that operation indications are known, the treatment of serious ocular injury with lens dislocation with vitrectomy and ICL implantation is practical. The two kinds of lens have merits and demerits respectively and should be selected as per the conditions of patients.
