胃癌术后病理特征回顾性分析

摘　　要：目的回顾性手术治疗胃癌患者临床病理资料,为胃癌的临床诊治提供指导帮助。方法对自2009年1月至2015年12月于我院行手术切除治疗的490例胃癌患者一般情况及术后病理进行回顾性分析。结果术后证实早期胃癌共138例,占同期胃癌手术患者的28.16%,其中T1a期77例,T1b期61例。早期胃癌不论在病变的大小及淋巴结转移、脉管浸润和神经浸润比率上均显著低于进展期胃癌(P<0.05)。早期胃癌中T1a期患者的病变的大小及淋巴结转移、脉管浸润和神经浸润比率上均显著低于T1b期患者。分化型胃癌中早癌比率显著高于未分化型胃癌且分化型早癌的淋巴结转移率显著低于未分化型。结论早期胃癌尤其是黏膜内癌的转移及复发风险明显低于进展期胃癌,分化型早癌比率明显高于未分化型且转移风险较低。Objective Retrospective analysis for clinical and post-operation histopathology results of gastric cancer and to provide the direction in clinical for the choice of treatment. Methods From January 2009 to December 2015,clinicopathologic factors including age,gender,tumor size,location,depth of tumor invasion and vascular involvement of 490 gastric cancer patients who were surgical resected in our hospital were retrospectively analyzed. Results 138 of 490 patients（ 28. 16%） were confirmed as early gastric cancer including 77 cases of T1 a and 61 cases of T1 b. The incidence of lymph node metastasis,vascular and neural involvement were significantly lower in early gastric caner than advancer cancer（ P 0. 05）. Furthermore,the lesion size,lymph node metastasis,vascular and neural involvement of T1 a caner was significantly lower than T1 b. The rate of early cancer in differentiated cancer was significantly higher than undifferentiated cancer.Conclusions The rate of differentiated early carcinoma is significantly higher than that of undifferentiated type,and the risk of metastasis is lower
