老年晚期消化道肿瘤患者X线下胃肠营养管置入术疗效分析

摘　　要：目的探讨X线下介入放置胃肠营养管用于老年晚期消化道肿瘤患者营养支持的效果。方法将50例老年晚期消化道肿瘤患者随机分为2组:观察组25例X线下介入放置胃肠营养管行肠内营养支持;对照组25例通过静脉通路给予肠外营养。治疗1个月后比较2组血红蛋白(hemoglobin,Hb)、白蛋白(Albumin,Alb)及免疫球蛋白水平变化。比较2组治疗过程中出现并发症的情况。结果 2组治疗后Hb、Alb以及免疫球蛋白Ig A、Ig G、Ig M均较治疗前升高(P<0.05),观察组上述指标升高幅度较对照组更大,治疗后观察组Hb、Alb、免疫球蛋白Ig A、Ig G高于对照组(P<0.01),观察组生存时间明显长于对照组(P<0.05)。观察组治疗过程中无并发症发生,对照组有7例出现并发症(P<0.05)。结论对老年晚期消化道肿瘤患者行X线下胃肠营养管置入术营养支持,可有效保护其胃肠道功能、改善其营养状况、提高其免疫力。Objective To investigate the effect of nutritional support in the treatment of elderly patients with advanced digestive tract cancer by interventional placement of gastrointestinal tract under Xray. Methods Fifty patients with late digestive tract tumor were randomly divided into 2 groups：Observation group of 25 cases with X-ray placed in jejunum nutrition tube enteral nutrition support and control group 25 cases to parenteral nutrition. After one month treatment,changes of blood albumin,hemoglobin and immunoglobulin levels in two groups werevcompared. The complications of the 2 groups were compared. Results Two groups after treatment albumin、hemoglobin and immunoglobulin,Ig A,Ig M and Ig G were higher than before treatment（ P〈0. 05）. Compared with the control group,the above indexes of the observation group were significantly higher. Observation group after treatment of albumin、hemoglobin and immunoglobulin,Ig A,Ig G,higher than the control group（ P〈0. 01）,The survival time of patients in the observation group appear longer than the control group（ P〈0. 05）. The observation group had no complications during the treatment,and the control group had 7 cases of complications（ P〈0. 05）. Conclusion Elderly patients with advanced digestive tract cancer were treated with nutritional support,which can effectively protect the gastrointestinal function,improve the nutritional status and improve the immune function of the patients with advanced gastrointestinal cancer.
