开腹与腹腔镜胃癌根治术的术后恢复情况及患者生存率对比

摘　　要：目的对比开腹与腹腔镜胃癌根治术的术后恢复情况及患者生存率。方法 386例早期胃癌患者,随机分为腹腔镜组(306例)与开腹组(80例)。腹腔镜组采用腹腔镜胃癌根治术进行治疗,开腹组采用传统开腹手术进行治疗。比较两组患者的首次进食时间、下床时间、排气时间、住院时间及病理疼痛评分,并对患者术后第1、3、5年的生存率进行分析。结果腹腔镜组首次进食时间(4.12±1.92)d、下床时间(1.47±0.81)d、排气时间(1.67±0.43)min均明显早于开腹组(6.32±1.65)d、(2.72±0.68)d、(4.12±1.28)min(P<0.05),腹腔镜组住院时间(11.10±3.31)d明显短于开腹组(16.24±4.55)d(P<0.05);腹腔镜组患者术后视觉模拟评分法(VAS)评分优于开腹组(P<0.05);两组第1、3、5年生存率比较差异均无统计学意义(P>0.05)。结论腹腔镜胃癌根治术有益于早期胃癌患者的术后恢复,但对提高患者生存率上并无较大贡献。ObjectiveTo compare postoperative recovery and survival rate between laparotomy and laparoscopic radical operation for gastric cancer.MethodsA total of 386 patients with early gastric cancer were randomly divided into laparoscope group （306 cases） and laparotomy group （80 cases）. The laparoscope group received laparoscopic radical operation for gastric cancer, and the laparotomy group received conventional laparotomy for treatment. Comparison was made on first meal time, off-bed time, exhaust time, hospital stay time and pathological pain score between the two groups, along with analysis of postoperative 1, 3, 5 years survival rates in patients.ResultsThe laparoscope group had all earlier first meal time as （4.12±1.92） d, off-bed time as （1.47±0.81） d, and exhaust time as （1.67±0.43） min than （6.32±1.65） d, （2.72±0.68） d and （4.12±1.28） min of the laparotomy group （P〈0.05）. It also had shorter hospital stay time as （11.10±3.31） d than （16.24±4.55） d of the laparotomy group （P〈0.05）. The laparoscope group had better postoperative visual analogue scale （VAS） score than the laparotomy group （P〈0.05）. There was no statistically significant difference of 1, 3, 5 years survival rates between the two groups （P〉0.05）.ConclusionLaparoscopic radical operation for gastric cancer is beneficial to postoperative recovery in early gastric cancer patients, while this method contributes little to survival rate improvement.
