青光眼的改良手术疗效分析

摘　　要：目的探讨改良巩膜瓣联合可松解缝线技术治疗青光眼的疗效。方法 46例（62只眼）原发性急性闭角型青光眼患者,随机分为改良手术组23例（33只眼）和传统手术组23例（29只眼）。局部麻醉下分别行改良巩膜瓣联合可松解缝线抗青光眼手术和传统小梁切除术。术后随访6~12个月,平均随访7.8个月,记录患者视力、眼压、前房形成、滤过泡及其他并发症等情况。结果改良手术组术后视力恢复优于传统手术组（P〈0.05）;远期眼压（13.8±3.7）mm Hg（1 mm Hg=0.133 k Pa）、功能性滤过泡形成率93.9%亦优于传统手术组（16.9±4.7）mm Hg、65.5%（P〈0.05）。结论临床选择改良巩膜瓣联合可松解缝线技术治疗青光眼,具有术中操作简便、术后视力恢复明显、降眼压效果较好、前房较稳定、滤过泡形成好、并发症少等优点,是可行的优于传统小梁切除术的手术方法。Objective To investigate curative effect by modified sclera flap combined with releasable suture in the treatment of glaucoma.Methods A total of 46 patients （62 eyes） with primary acute angle-closure glaucoma were randomly divided into modified operation group with 23 cases （33 eyes） and traditional operation group with 23 cases （29 eyes）. They respectively received modified sclera flap combined with releasable suture antiglaucoma operation and traditional trabeculectomy under local anesthesia. Postoperative follow-up lasted for 6~12 months, with mean time as 7.8 months, to record vision, intraocular pressure, anterior chamber formation, filtering bleb and complications.Results The modified operation group had better postoperative vision recovery than the traditional operation group （P〈0.05）. It had better long-term intraocular pressure as （13.8±3.7） mm Hg （1 mm Hg=0.133 kPa） and formation rate of functional filtering bleb as 93.9% than （16.9±4.7） mm Hg and 65.5% of the control group （P〈0.05）.Conclusion Clinical implement of modified sclera flap combined with releasable suture in treating glaucoma contains advantages of easy operation, remarkable recovery of postoperative vision, good intraocular pressure reducing, stable anterior chamber, good filtering bleb formation and few complications. This method is feasible and also better than traditional trabeculectomy.
