摘  要:目的探讨乳腺癌组织中ER、PR、C-erb B-2、P53和Ki-67的表达与乳腺癌临床特点的关系。方法应用免疫组化法检测内蒙古自治区人民医院2008年1月至2014年12月确诊的1186例乳腺癌组织中上述免疫组化指标的表达量,及其与临床病理参数的关系,结果做统计学分析。结果在1 186例乳腺癌患者中,(1)ER、PR、C-erb B-2、P53和Ki-67阳性率分别为60.4%(716/1 186)、63.4%(752/1 186)、54.5%(646/1186)、56(664/1 186)、69.9%(829/1 186)。(2)中年组(﹥35岁,≤60岁)ER+/PR+例数及C-erb B-2+/P53+/Ki-67+例数较青年组(≤35岁)、老年组(﹥60岁)多。(3)与其他病理类型相比,浸润性导管癌中ER/PR阳性率和C-erb B-2/P53/Ki-67阳性率较高,差异有统计学意义。(4)PR和ER与乳腺癌组织分化程度呈负相关,P53、C-erb B-2和Ki-67与乳腺癌组织分化程度呈正相关。结论检测ER、PR、C-erb B-2、P53和Ki-67指标有助于判断乳腺癌的恶性程度和生物学行为,为乳腺癌的个性化治疗提供依据。Objective To investigate the correlation between expressions of ER, PR, C-erb B-2, P53 and Ki-67 and clinicopathological characteristics in breast cancer. Methods Using immunohistochemical assay, the expressions of ER, PR, C-erb B-2, P53 and Ki-67 in 1186 cases of breast cancer were detected, all of which were pathologically diagnosed in the Inner Mongolia People's Hospital from 2008 January to 2014 December. The correlation between expressions of ER, PR, C-erb B-2, P53 and Ki-67 and clinicopathological characteristics were retrospectively analyzed. Results(1) The positive rates of ER, PR, C-erb B-2, P53 and Ki-67 were 60.4%(716/1 186), 63.4%(752/1 186), 54.5%(646/1 186), 56%(664/1 186) and 69.9%(829/1 186).(2) The case numbers of ER+/PR+ and C-erb B-2+/P53+/Ki-67+ in the middle age group(﹥35, ≤60) were more than the youth group and the elderly group.(3) Compared with other pathologic types, the positive rates of ER+/PR+ and C-erb B-2/P53/Ki-67 in infiltrating ductal carcinoma were statistically significantly higher.(4) The positive rates of PR and ER were negatively correlated with the degree of differentiation in breast cancer, and in contrast, the positive rates of P53, C-erb B-2 and Ki-67 were positively correlated with the degree of tumor differentiation. Conclusion The detection of these protein expressions in breast cancer is beneficial to understand the biological behavior of breast cancer, and significant for individualized treatment and prognostic evaluation.
