不典型毛细胞型星形细胞瘤的MRI表现

摘　　要：目的探讨颅内不典型毛细胞型星形细胞瘤的MRI特征，旨在提高对该病的认识。方法回顾性分析经手术病理证实的13例原发性毛细胞型星形细胞瘤患者影像学资料，其中儿童6例（4～13岁），成人7例（23～54岁），平均年龄29．45岁。结果病变数目、部位：单发11例，多发2例，共有毛细胞型星形细胞瘤17个。发生于幕下7例，幕上6例。MRI表现：病灶实性部分T1WI呈等或稍低信号，T2WI呈稍高信号，囊变或坏死部分．T1WI呈低信号，T2WI呈高信号。病灶多呈实性或囊实性类圆形肿块，实性4例，囊性或囊实性9例，增强扫描囊壁及壁结节多呈明显均匀或不均匀强化。13例中，1例伴瘤内出血及钙化，7例伴幕上脑室扩张，4例伴瘤周轻度水肿。结论对于颅内不典型毛细胞型星形细胞瘤的诊断及术前评估，MRI扫描检查具有重要意义。Objective To investigate MR characteristics of atypical piloeytic astroeytoma （PA）, so as to improve the further understanding of the disease. Methods MR features and preoperative diagnostic accuracy were retrospectively analyzed of 13 patients with pathologically proven PA after surgery. Among them, children and adults were 6 cases （ aged 4- 13 ） and 7 cases （23 54） respectively. The average age was 29.45 years old. Results Number of lesions and parts ： 11 cases were single, 2 cases were multiple, A total of 17 PA were found. 7 lesions were located infrat-entorial and 6 lesions were located supratentorial. MR fealures ： solid lesions showed heterogeneous hypo or isointensity on Tl WI, hyperintensity on T2 WI . As for the Capsule or necrosis part, T1 WI was low signal and T2 WI was high signal. 4 lesions were solid type and 9 lesions were cystic cystic solid type. The solid part of tumors showed obviously homogeneous or heterogeneous enhancement with sharp edge. Among them, 1 case presented with bleeding and calcification, 7 cases with supratentorial ventricles expansion, 4 cases with mild edema around tumor. Conclusion Understanding the MRI appearances of atypical pilocytic astrocytom at unfrequent sites is very important in characterization of the tumors, moreover, it can provide more information before surgery.
