囊变在腮腺病变MRI鉴别诊断中的价值

摘　　要：目的探讨瘤内囊变特点对腮腺良、恶性病变的鉴别诊断价值。方法回顾性分析67例均经病理证实的腮腺MRI图像，其中良性病变50例、恶性17例，观察MRI图像中腮腺病灶内囊变特点，运用SPSS16．0统计学软件对相关图像数据进行卡方检验分析，研究良、恶性病变内囊变发生率、囊变区分布、囊变区所占肿物比例分级、囊变区内信号分级特点。结果囊变发生率在腮腺良、恶性病变间差异不存在统计学意义（x2=0．062，P=0．803）；囊变区分布（X2=17．484，P=0．000）、囊变区所占肿物比例分级（X2=21．486，P=0．000）、囊变区内信号分级（X2=7．370，P=0．007）在腮腺良、恶性病变间差异具有统计学意义，对有意义特点进一步行ROC曲线分析，其鉴别腮腺良恶性病变的ROC曲线下面积（AUC）值分别为0．852、0．834、0．734，其诊断敏感性分别为90．9％、72．7％、90．9％，诊断特异性分别为79．4％、94．1％、55．9％。结论MRI显示的囊变特点对腮腺良、恶性病变的鉴别诊断有一定参考价值。Objective To explore the diagnostic performance of intratumoral cyst on MRI in distinguishing parotid lesions from benign to malignant. Methods We retrospectively evaluated the preoperative MRI findings of 67 patients with malignant and benign parotid lesions by surgical pathology, including the prevalence of cyst, distribution, occupying rate and MR signal intensity of intratumoral cyst, which were analyzed by Chi-square test and receiver operating characteristics （ROC） curve analysis with SPSS 16.0 software. Results The prevalence of cystic （X2 = 0. 062, P = 0.803 ） was insignificantly different between malignant and benign lesions, while, the distribution （ X2 = 17. 484, P = 0.000） , occupying rate （ X2 = 21. 486, P = 0. 000） and MR signal intensity of intratumoral cyst （ X2 = 7. 370, P = 0. 007 ） were significantly different between malignant and benign lesions. Area under the ROC curves were 0. 852, 0. 834 and 0. 734, respectively. The Sensitivity was 90.9% , 72.7% and 90.9% , respectively, while, the specificity was 79.4% , 94.1% and 55.9% , respectively. Conclusion Intratumoral cyst on MR imaging may have certain value in distinguishing parotid lesions from malignant to benign.
