vWF与GMP-140在aSAH患者中的动态变化及意义

摘　　要：目的：探讨血管性假血友病因子（vWF）与血小板膜糖蛋白-140（GMP-140）在动脉瘤性蛛网膜下腔出血（aSAH）患者中的动态变化及意义。方法：选取在四川省自贡市第四人民医院神经内科住院的aSAH患者60例,比较不同动脉瘤直径,不同Hunt-Hess分级（H-H分级）和不同预后结果的血浆中vWF和GMP-140动态变化,并分析血浆vWF和GMP-140的相关关系。结果：动脉瘤直径≥10mm的aSAH患者在出血后第1天和第4天vWF和GMP-140水平均高于动脉瘤直径〈5mm和动脉瘤直径5~10mm的患者（均P〈0.05）;H-H分级为Ⅵ~Ⅴ级的aSAH患者在出血后第1天和第4天vWF和GMP-140水平均高于Ⅰ~Ⅲ级的患者（均P〈0.05）;H-H分级为Ⅵ~Ⅴ级的aSAH患者在出血后第10天vWF和GMP-140水平高于Ⅰ级的患者（均P〈0.05）;预后不良的aSAH患者在出血后第1天GMP-140水平[（60.59±23.45）μg/L]高于预后良好的患者[（39.43±16.17）μg/L]（P〈0.05）;预后不良的aSAH患者在出血后第4天vWF[（3.08±1.57）U/mL]高于预后良好[（1.83±1.02）U/mL]的患者（P〈0.05）;预后不良的aSAH患者在出血后第10天vWF[（3.74±1.96）U/mL]和GMP-140[（43.51±14.27）μg/L]均高于预后良好的患者（P〈0.05）;第1天血中vWF和GMP-140呈正相关关系（r=0.621,P=0.0001）;第4天血中vWF和GMP-140呈正相关关系（r=0.282,P=0.029）。结论：血中的vWF和GMP-140的动态变化与动脉瘤直径、病情以及预后有关。Objective To investigate the dynamic changes of von Willebran factor （vWF） and platelet membraneglycoproteins-140 （GMP-140） and their clinical significance in patients with aneurysmal subarachnoidhemorrhage （aSAH ）. Methods： 60 cases of patients with aSAH treated in our hospital were selected. Thedynamic changes of vWF and GMP-140 from patients in plasma of different aneurysm diameters , Hunt-Hess （H -H ） grades and prognosis were compared and the relationship between vWF and GMP-140 in plasmawere analyzed. Results： Compared with the patients with aneurysm diam eter^ 10 m m , the vWF andGMP-140 levels in patients with diameter 5mm and between 5-10 mm were elevated on the 1st and 4thdays after the hemorrhage （ P 〈0. 05）. The vWF and GMP-140 levels in class VI to V （H -H grade） patientswere higher than those in class I to III patients on the 10t.h day after the hemorrhage （ P 〈0. 05）.Poor prognosis patients Had a higher level of GMP-140 on the 1st day （ P 〈0. 05） , and a higher level ofvWF on the 4t.h day （ P 〈0. 05）. And on the 10th day after the hemorrhage, prognosis patients Had higherlevels in both vWF and GMP-140 （ P 〈 0 . 05）. vWF and GMP-140 were positively correlated on the 1st and4t.h days （ P 〈0. 05）. Conclusion： The dynamic changes of vWF and GMP-140 in plasma were related tothe aneurysm diameter, the condition of illness and the prognosis.
