摘　　要：目的研究鼻腔小圆细胞性恶性黑色素瘤临床病理特征及其鉴别诊断。方法收集12例鼻腔小圆细胞性恶性黑色素瘤,通过组织学和免疫组化进行观察。结果鼻腔小圆细胞性恶性黑色素瘤细胞呈弥漫、片状或巢状分布,瘤细胞形态较为一致,瘤细胞小、圆形、胞质稀少,无色素。大部分病例肿瘤细胞围绕小血管呈血管外皮瘤样排列。免疫组化vimentin、S-100、HMB45和melan-A为(+)。结论鼻腔小圆细胞性恶性黑色素瘤罕见,需要通过组织学和免疫组化染色特点作出正确的诊断,并同鼻腔其他小圆细胞性恶性肿瘤鉴别。Objective To study the clinicopathological features, the diagnosis and differential diagnosis of small round cell malignant melanoma in the nasal cavity. Methods The clinical data, histological features, and immunohistochemical staining were investigated in 12 cases of small round cell malignant melanoma in the nasal cavity. Results The histological features of small round cell malignant melanoma were manifested as sheets and nests of amelanotic cells, seemingly undifferentiated small round cells. The most tumors were arranged in a peritheliomatous distribution. Immunohistochemical studies confirmed the diagnosis of small round cell malignant melanoma with positive reactions for vimentin, S-100, HMB45, and melan-A. Conclusion Small round cell malignant melanoma of nasal cavity is rare. It needs to be considered correct diagnosis and differential diagnosis based on the differential histopathological and immunohistochemical characteristics of sinonasal small cell malignant tumors.
