急性白血病患者医院感染的危险因素分析

摘　　要：目的分析急性白血病患者医院感染的危险因素,为提高治疗效果及改善预后提供参考依据。方法选取2012年3月-2014年3月于医院接受治疗的急性白血病患者106例为研究对象,采用回顾性分析方式调查其医院感染的相关因素,数据采用SPSS 18.0软件进行统计分析。结果 106例急性白血病患者发生医院感染34例,医院感染率为32.07%;共分离出56株病原菌,其中革兰阴性菌32株占57.14%,以肺炎克雷伯菌和大肠埃希菌为主,革兰阳性菌21株占37.50%,真菌3株占5.36%;多因素logistic回归分析显示,患者年龄、住院季节、侵入性操作、抗菌药物的使用、激素的使用、化疗周期、白细胞计数、住院时间等因素是急性白血病患者发生医院感染的独立危险因素(P<0.05)。结论临床应尽量减少急性白血病患者医院感染的危险因素,采取有效措施控制,以降低医院感染率,提高治疗效果并改善预后。OBJECTIVE To analyze risk factors of hospital infections in patients with acute leukemia,so as to provide reference for improving treatment and prognosis.METHODS A total of 106 cases of acute leukemia patients treated in the hospital from Mar.2012 to Mar.2014 were selected and analyzed by retrospective survey about the related factors of hospital infections.SPSS 18.0was used for data analysis.RESULTS There were 34 cases of hospital infections in the 106 cases of acute leukemia patients,and the hospital infection rate was 32.07%.There were 56 cases of pathogens were isolated,including 32 strains of gram-negative bacteria,accounting for 57.14%,mainly Klebsiella pneumoniae and Escherichia coli,and 21 strains of gram-positive bacteria,accounting for37.50%,3strains of fungi,accounting for 5.36%.Age,hospitalization seasons,invasive procedures,use of antibiotics,use of hormones,chemotherapy cycles,white blood cell count,length of stay were the independent risk factors of hospital infections in patients with acute leukemia（P〈0.05）.CONCLUSION The risk factors causing hospital infections in patients with acute leukemia in clinic should be reduced by taking effective measures to control,so as to decrease the hospital infection rate and improve treatment and prognosis.
