血清HE4和CA125联合检测卵巢上皮性癌的诊断价值

摘　　要：目的探讨血清HE4和CA125联合检测卵巢上皮性癌患者的临床诊断应用价值。方法运用酶联免疫检测方法检测血清HE4、化学发光法测定血清CA125;选择卵巢上皮性癌116例为恶性肿瘤组、卵巢良性肿瘤60例为良性肿瘤对照组。另选同期进行健康体检的正常女性50例为健康对照组。比较3组患者HE4和CA125水平,分析联合检测与单独检测的灵敏度、特异度、准确度、阳性及阴性预测值。结果与健康对照组比较,恶性肿瘤组患者血清HE4和CA125水平明显提高,差异有统计学意义(P<0.05)。与良性肿瘤对照组患者比较,恶性肿瘤组患者血清HE4和CA125水平明显提高,差异有统计学意义(P<0.05)。与HE4或CA125单一检测结果比较,CA125和HE4联合检测的灵敏度、特异度、准确度、阳性预测值和阴性预测值均明显提高,分别达到88.89%、92.00%、89.77%、96.55%和76.67%,差异有统计学意义(P<0.05)。结论血清HE4和CA125联合检测对于卵巢上皮性癌患者的诊断具有较高价值。Objective To explore the diagnostic value of combination detection of serum human epididymis protein （HE4） with ovarian carbohydrate antigen 125 （ CA125 ） in ovarian epithelial carcinoma. Methods The serum levels of HFA were detected by ELISA,and serum levels of CA125 were detected by chemil 116 patients with ovarian epithelial carcinoma were served as malignant tumor group, and 60 patients with benign ovarian tumors were served as benign tumor control group, moreover, the other 50 healthy subjects were served as heahh control group. The levels of HE4 and CA125 were observed and compared among the three groups, furthermore, the sensitivity, specificity, accuracy, positive and negative predictive values of combination detection and single detection were analyzed. Results As compared with those in health control group and benign tumor control group, the levels of serum HFA and CA125 in malignant tumor group were significantly increased （ P 〈 0.05）. As compared with those detected by single detection method, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value of CA125 and HE4 by combination detection were significantly increased, which reached 88.89%, 92.00%, 89.77%, 96.55% ,76.67%, respectively （ P 〈 0.05 ）. Conclusion The combination detection of CA125 with HE4 plays an important role in diagnosis of ovarian epithelial carcinoma.
