摘  要:目的探讨椎基底动脉压迫型前庭阵发症的磁共振影像学特征。方法共收集前庭阵发症(vestibular paroxysmia,VP)患者64例,分为非椎基底动脉压迫型组(对照组)32例和椎基底动脉压迫型组(观察组)32例,应用三维时间飞跃法磁共振血管成像技术,判断64例患者的前庭蜗神经周围有无神经血管交互压迫(neurovascular cross-compression,NVCC)及其压迫类型,并对两组间NVCC压迫点至前庭蜗神经出脑干处的最短距离、基底动脉及双侧椎动脉血管直径、双侧椎动脉间血管直径变异度进行统计学分析。结果在观察组NVCC压迫点距前庭蜗神经出脑干处距离为(6.26±2.02)mm明显小于对照组(8.17±1.98)mm(t=-3.872,P=0.000,P<0.01);而观察组椎动脉血管直径(3.53±0.42)mm与对照组(3.44±0.31)mm无明显差异(t=0.962,P=0.340,P>0.05);两组的基底动脉血管直径观察组(3.74±0.41)mm与对照组(3.65±0.29)mm间亦无统计学意义(t=0.992,P=0.326,P>0.05);而观察组的双侧椎动脉血管直径变异度(1.09±0.31)mm明显高于对照组(0.82±0.35)mm(t=3.330,P=0.001,P<0.05)。结论椎基底动脉压迫型VP患者NVCC压迫点距前庭蜗神经出脑干处距离较近且双侧椎动脉血管直径变异度较大。Objective To explore the MRI characteristics of vestibular paroxysmia ( VP) caused by vertebrobasilarartery compression. Methods 64 cases of VP were collected and further divided into vertebrobasilar artery compressiongroup (observation group, 32 cases) and non-vertebrobasilar artery compression group (control group, 32 cases). Three dimensionaltime of flight magnetic resonance angiography(3D-T0F-MRA) scan were carried out in all the cases,and whetherthere was neurovascular cross-compression ( NVCC) and the subtypes of NVCC were judged by a professional radiologist.Furthermore the distance between NVCC site and the brainstem, the diameter of basilar artery and both vertebral artery andthe variation of bilateral vertebral artery diameter were also collected and analyzed. Results The distance between NVCCsite and brainstem in observation group (6. 26 ± 2. 0 2 )mm was significantly shorter than control group (8. 17 ± 1 .9 8 ) mm(t = -3 . 872,P =0. 000,P 〈 0 .0 1 ) ; while there was no difference between the two groups in vertebral artery diameterwhich was ( 3.53 ± 0. 42 ) mm in observation group and ( 3. 44 ± 0. 31) mm in control group respectively ( 艺=0. 962 , P =0. 340,P 〉 0. 05 ) ; And there was no difference in the diameter of basilar artery between observation group ( 3. 74 ± 0. 41)mm and control group( 3. 65 ± 0. 29) mm( t=0. 992,P = 0. 326,P 〉 0. 05 ) either; But the bilateral vertebral artery diameterdifferent in observation group ( 1.09 ± 0. 31) mm was significantly higher than of the control group ( 0. 82 ± 0. 35 ) mm ( t =3. 3 3 0 = 0. 001 〈0. 05) . Conclusions The distance between NVCC site and brainstem in vertebrobasilar artery compressionsubtype of VP was the closer and the differences of bilateral vertebral artery diameter between was larger comparedto non-vertebrobasilar artery compression VP.
