分化型甲状腺癌术后^(131)I清甲疗效相关影响因素分析

摘　　要：目的:探讨分化型甲状腺癌术后^(131)I清甲疗效相关影响因素。方法:回顾性分析分化型甲状腺癌术后^(131)I清甲治疗患者128例临床资料,总结包括年龄、性别、病理分型、手术切除方式、肿瘤转移、肿瘤转移部位、血清促甲状腺激素(TSH)水平及^(131)I治疗时间等临床特征对疗效的影响。结果:对不同性别、年龄、病理分型及肿瘤转移有无进行分组比较,分化型甲状腺癌患者术后^(131)I治疗临床疗效差异无统计学意义(P>0.05);而行甲状腺全切、肿瘤颈部淋巴结转移、血清TSH水平20~40mI U/L及^(131)I治疗时间≤6个月组分化型甲状腺癌患者术后^(131)I清甲临床疗效显著优于其他分组,差异有统计学意义(P<0.05);手术切除方式、血清TSH水平及^(131)I治疗时间是影响分化型甲状腺癌术后^(131)I治疗效果独立因素。结论:分化型甲状腺癌术后^(131)I清甲疗效与手术切除方式、血清TSH水平及^(131)I治疗时间具有相关性。Objective： To investigate influencing factors of therapeutic effects of differentiated thyroid carcinoma treated by^131I for thyroid ablation. Methods： The clinical data of 128 patients with differentiated thyroid carcinoma treated by operative section and^（131）I therapy for thyroid ablation after operation were retrospectively analyzed to summarize the influence of clinical characteristic on therapeutic effects of differentiated thyroid carcinoma treated by^131I including age,gender,pathological type,methods of operative section,carcinoma metastasis,location of carcinoma metastasis,levels of serum TSH and treatment time of^131I. Results： There was no significant difference in therapeutic effects of differentiated thyroid carcinoma treated by using^131I of different group by age,gender,pathological type and carcinoma metastasis（ P〈0. 05）. The therapeutic effects of differentiated thyroid carcinoma treated by^131I for thyroid ablation of group with total thyroid section,carcinoma metastasis of neck lymph node,levels of serum TSH for 20 ~40m IU / L and treatment time of^131I for ≤6 months was significant better than other group（ P〈0. 05）. The independent influencing factors on therapeutic effects of differentiated thyroid carcinoma treated by using^131I included methods of operative section,levels of serum TSH and treatment time of^131I. Conclusion： The factors including methods of operative section,levels of serum TSH and treatment time of^131I could influence therapeutic effects of differentiated thyroid carcinoma treated by^131I.
