中老年精神障碍无抽搐电休克治疗后血压和心率变化特征研究

摘　　要：目的探讨中老年精神障碍患者无抽搐电休克治疗(MECT)后心率、血压变化的特征。方法选取32例中老年精神障碍患者(研究组)和115例青年精神障碍患者(对照组)接受MECT治疗,比较两组患者MECT麻醉前、麻醉后、发作后、发作后15 min、发作后30 min、发作后60 min的收缩压(SBP)、舒张压(DBP)、心率(HR)的变化情况。结果两组患者麻醉后SBP、DBP均较麻醉前下降,HR增快(P<0.05),发作后SBP、DBP、HR均升高(P<0.05)。研究组患者麻醉后、发作后SBP、DBP的变化程度均高于对照组(P<0.05)。发作后30 min研究组患者SBP高于麻醉前(P<0.05)。发作后60 min两组患者SBP、DBP和HR均恢复到麻醉前水平。结论中老年患者MECT治疗过程中血压和心率变化幅度较大。Objective To explore the changes of blood pressure and heart rate in middle aged and elderly patients with mental disorders after modified electroconvulsive therapy （MECT）. Methods A total of 32 middle aged and elderly patients with mental disorders in study group and 115 young patients with mental disorders in control group were treated with MECT and assessed with systolic blood pressure （ SBP）, diastolic blood pressure （DBP） and heart rate （HR） at pre-anesthesia, post- anesthesia, post-seizure, 15 minutes after seizure, 30 minutes after seizure and 60 minutes after seizure. Results The SBP and DBP in two groups at post-anesthesia were significantly lower than those at pre-anesthesia（ P 〈 0.05 ）, and the rates of heart at post-anesthesia were significantly faster than those at pre-anesthesia（P 〈 0.05）. The SBP, DBP and HR in two groups at postseizure were significantly higher than those at pre-anesthesia （ P 〈 0 05 ）. At post-anesthesia and post-seizure, the changes of SBP and DBP in study group were significantly more than those in control group （ P 〈0. 05 ）. In study group, the SBP at 30 minutes after seizure was significantly higher than that at pre-anesthesia（ P 〈0. 05 ）. There was no difference for SBP, DBP and HR in two groups between pre-anesthesia and 60 minutes after seizure （ P 〉 0.05 ） ）. Conclusion There are obvious changes of blood pressure and heart rate in middle aged and elderly patients with mental disorders after MECT.
