急性非静脉曲张性上消化道出血应用十灰散治疗临床疗效观察

摘　　要：目的:分析和研究急性非静脉曲张性上消化道出血应用十灰散治疗的临床疗效。方法:纳入2013年7月—2015年1月急性非静脉曲张性上消化道出血患者100例,按照随机数字表法随机分为中医组与西医组,每组各有患者50例。所有患者行常规对症支持和干预,西医组患者辅以奥美拉唑进行止血;中医组患者辅以十灰散进行止血,对比两组止血效果、72 h再出血率、用药不良作用发生率、平均止血时间、平均住院时间、治疗前后症状评分的变化。结果:两组患者止血效果相似,差异经χ2检验显示无统计学意义(P>0.05)。中医组72 h再出血率、用药不良作用发生率明显低于西医组,差异经χ2检验有统计学意义(P<0.05)。两组患者治疗前症状评分相似,差异经t检验显示无统计学意义(P>0.05)。中医组患者治疗后症状评分明显低于西医组,且平均止血时间、平均住院时间明显短于西医组,差异经t检验显示有统计学意义(P<0.05)。结论:急性非静脉曲张性上消化道出血应用十灰散治疗的临床疗效确切,可在安全用药的基础上有效止血,促进患者胃痞、胃痛、纳差等症状的改善,缩短止血和住院时间,改善患者预后,值得推广。Objective：To analyze and study the clinical effect of Ten Grey Powder on the treatment of acute non varicose upper gastrointestinal bleeding. Methods：100 cases of patients with acute non varicose upper gastrointestinal bleeding in January 2015 and July 2013 were enrolled. The patients were randomly divided into the TCM group and the western medicine group according to the random number table. 50 cases were in each group. All patients were treated with conventional symptomatic support and intervention,patients with western medicine group were treated with omeprazole to stop bleeding;traditional Chinese medicine group patients were treated with Ten Grey Powder for hemostasis,compared with two groups of hemostatic effect,72 hours bleeding rate,the incidence of adverse drug effects,average hemostasis time,the average hospitalization time,before and after the treatment of symptom scores. Results：The two groups of patients with similar hemostatic effects,the difference was not statistically significant（P〈0.05）. Traditional Chinese medicine group 72 hours of bleeding rate,incidence of adverse drug effects was significantly lower than that of Western medicine group,the difference was statistically significant（P〈0.05）by χ2 test. The two groups of patients before treatment symptom scores were similar,the difference was not statistically significant（P〈0.05）after t test. The symptom score of TCM group was significantly lower than that of Western medicine group,and the average hemostasis time and average hospitalization time were significantly shorter than that of Western medicine group,the difference was statistically significant（P〈0.05）,the difference was statistically significant（P〈0.05）. Conclusion：The application of Ten Grey Powder in acute non-variceal gastrointestinal bleeding has definite clinical effect,could promote improvement in patients with stomach flatulence,stomach pain,anorexia and other symptoms,shorten the bleeding and hospitalization time,improve the prognosis of pati
