小关节植骨融合术和横突间植骨融合术治疗胸腰椎爆裂性骨折的临床疗效对比

摘　　要：目的:对比小关节植骨融合术和横突间植骨融合术治疗胸腰椎爆裂性骨折的临床疗效。方法:随机选取实验组患者和对照组患者各40例,实验组采用小关节植骨融合术进行治疗,对照组采用横突间植骨融合术进行治疗。比较手术时间、术中出血量等临床指标;比较术前、术后1周及内固定取出后6个月的脊柱矢状位Cobb角、椎体前缘压缩率等影像学指标;比较VAS评分并观察断钉断棒、失稳、继发后凸畸形等并发症发生情况。结果:两组患者均较顺利地完成手术。两组患者治疗前、术后1周及内固定取出后6个月的脊柱矢状位Cobb角、椎体前缘压缩率比较,差异均无统计学意义(P>0.05)。实验组手术时间和术中出血量均低于对照组(P<0.05)。实验组VAS评分明显低于对照组(P<0.05)。两组断钉断棒、失稳、继发后凸畸形等并发症发生率比较,差异无统计学意义。结论:小关节植骨融合术和横突间植骨融合术治疗胸腰椎爆裂性骨折疗效相近,但小关节植骨融合术手术时间较短,术中出血量较少,疼痛程度较低。Objective：To compare the clinical effect of the facet joint fusion and intertransverse fusion of the operation on thoracolumbar burst fracture associated with spinal cord injury. Method：80 patients were randomly divided into experimental group （ 40） and the controlled group （ 40） . The patients in the experimental group were received the treatment of facet joint fusion, while the patients in the controlled group were administrated with intertransverse fusion. Compare the time and bleeding in the operational time between two groups. The compression ratio of anterior vertebral,spinal sagittal Cobb,VAS score and the adverse effect were also compared between two groups. Results：All patients were completed the operation successfully. There was no significant difference between two groups in the aspect of anterior vertebral, spinal sagittal Cobb （ P〉0. 05 ） . while the experimental group has less operational time、operational bleeding and VAS score,as compared to the controlled group（P〈0.05）.Conclusion：The clinical effects of two groups were similar,but the method of facet joint fusion was more significant in the treatment of throacolumbar burst fracture associated with spinal cord injury,because it has less has less operational time,operational bleeding and VAS score.
