白内障手术术源性散光测量及其影响因素分析

摘　　要：目的介绍白内障手术中术源性散光（SIA）的测量方法，探讨切口位置对SIA的影响。方法采用回顾性病例分析。选择2014年10月至2015年3月在解放军第180医院眼科就诊的31只眼白内障超乳手术患者，既往无眼表疾病，无眼部外伤或手术史。主切口位于135度，透明角膜切口，切口宽度2．8mm，由同一术者完成手术，超乳手术过程顺利并植入后房型人工晶状体，测量术前及术后3个月IOL Master，记录角膜散光陡峭轴及平坦轴的屈光力和轴向，登陆www．doctor—hill．com，下载网站的免费SIA计算器计算SIA。比较有眼颞上方切口和左眼鼻上方切口对SIA的影响。结果31只眼中，16只眼为右眼，手术切口位于颞上方；15只眼为左眼，手术切口位于鼻上方。SIA计算结果右眼为（0．36±0．19）D，左眼为（0．43±0．27）D，两者比较差异具有统计学意义（P〈0．05）。结论SIA测量方法简单易行，在计算散光晶状体植入轴向时，术者应输入个性化的SIA，而不应使用默认值0．50D，有眼颞上方手术切口的SIA小于左眼鼻上方切口。Objective To introduce a method calculating surgically induced astigmatism （SIA） and discuss the influence of incision location. Methods Retrospective case analysis of 31 eyes were recruited which undergoing emulsification and excluding ocular surface diseases, eye injuries or surgical history. The main incision was located at 135 which was a 2.8mm clear cornea incision. All operations were done by one surgeon and intraocular lens were implanted successfully. Both magnitude and direction of cornea astigmatism were measured by IOL Master before and 3 months after surgery. The cornea power and axial was recorded. Results Among the 31 eyes, 16 eyes were operated in temporal superior incision on the right eyes and the rest 15 eyes were in the nasal superior incision on the left eyes. The SIA of the right eyes was （0.36±0.19）D, while the SIA of the left eyes was （0.43±0.27）D. There were significant differences between the right and left eyes （P 〈0.05）. Conclusions It＇s easy to measure SIA. The individual SIA should be applied because the default of 0.5D is not accurate. The SIA in temporal superior incision on the right eyes is smaller than in nasal superior incision on the left eyes.
