摘 要:目的探讨Lenstar与A型超声测量超长眼轴高度近视患者眼轴长度差异性、相关性、一致性。方法临床病例对照研究。选取2014年8月至2015年8月在济南市明水眼科医院行白内障超声乳化人工晶状体植入术的超长眼轴高度近视患者97只眼为病例组,眼轴长度经Lenstar和接触式A型超声测量均大于28mm;另选取同时期行白内障超声乳化人工晶状体植入术的患者100只眼为对照组.眼轴长度经Lenstar和接触式A型超声测量均小于26IBm,所有患者均行Lenstar和A型超声测量眼轴长度,以配对f检验、Pearson相关和Bland—Ahman分别分析两种仪器测量眼轴长度的差异、相关性及一致性。结果对照组中Lenstar所测眼轴平均值较A超仅大(0.04±0.10)mm,但差异具有统计学意义(t=-4.020,P〈0.01);病例组Lenstar所测眼轴平均值较A超大(0.16±0.21)mm,差异具有统计学意义(t=-7.782,P〈0.01)。对照组和病例组两种仪器测量眼轴结果均差异有统计学意义(r=0.993,P〈0.001;r=0.985,P〈0.01);Bland—Altman图显示对照组测量一致性好,病例组一致性较差。结论Lenstar与A型超声相比在测量超长眼轴高度近视患者眼轴时一致性差,不宜替代使用。Objective To assess the difference, correlation and agreement of over-long axial length measurements with Lenstar and A-scan. Methods Ninety-seven eyes underwent phacoemulsification and IOL implantation with over-long axial length ≥28mm in our hospital from August 2014 to August 2015 were enrolled in the study (over-long axial length group), and 100 eyes underwent phacoemulsification and IOL implantation with axial length ≤26mm at the same period were enrolled as control, all patients were given axial length measurements both with Lenstar and A-scan, paired t test, Pearson correlation and Bland-Altman were used to analyze the difference, correlation and agreement of over-long axial length measurements with Lenstar and A-scan. Results The axial length was merely (0.04±0.10)mm longer with significant difference (t =-4.020, P 〈0.001) in the control group, in the over-long axial length group, it was (0.16±0.21)mm with significant difference (t =-7.782, P 〈0.001); there were a high correlation between the 2 instruments in both groups (r = 0.993, P 〈0.001; r =0.985, P 〈0.001); The Bland-Airman Plots displayed relatively good agreement in the control group, but poor in over-long axial length group. Conclusions Because of poor agreement between these devices in over-long axial length patients, the results of Lenstar and A-scan should not be exchanged.
